Position Purpose
The Clinical Staff Pharmacist will provide excellent customer service to patients and other members of the interdisciplinary team and serve as a clinical leader to students, residents, and other staff members. Actively participating in department programs and clinical/professional development projects, serve as a preceptor and mentor to pharmacy students on Introductory Pharmacy Practice Experience (IPPE) and Advanced Pharmacy Practice experience (APPE) rotations, primary preceptor to PGY-1 residents, assisting the Residency Program Director and serving on the Residency Advisory Committee. Will actively round on patients and develop a relationship with the interdisciplinary health care team, provide medication therapy plans monitor patients, and contribute to the creation/implementation of clinical services.
What you will do
Supervise non-pharmacist personnel by assigning duties, checking all orders, maintaining efficient workflow, and checking IVs and other medications prepared by technicians
Assess need for non-formulary items ordered according to department policy, and initiates contact with prescribing physician for substitute
Verify physicians’ orders for proper dose, regimen, allergies, potential drug-drug interactions/incompatibilities, adverse drug reactions and monitors lab data on all patients before dispensing medication. Utilizes appropriate communication log to notify physician/nurse of potential problem
Verify all narcotic orders are received, maintains perpetual inventory log, performs weekly C-II inventory and monthly controlled drug inventory and ensures all forms are completed and filed appropriately
Advise other health care professionals on drug interactions, incompatibilities, side effects, and proper administration of medications
Actively participate in department programs and projects
Minimum Qualifications
BS degree in Pharmacy, Pharm D degree preferred
Hospital experience and/or Residency preferred
CT Pharmacist license
Competency in providing direct patient care to neonate, pediatric, adolescent, young adult, adult and geriatric patients
Position Highlights and Benefits
Full time salaried at 40 hours per week.
Primarily days with some evening rotation, every third weekend and rotating Holidays
