The Pharmacist performs pharmaceutical care activities for customers/patients within the service area; monitors, documents and reports patient outcomes; provides or coordinates the dispensing of medications, home medical equipment and ancillary supplies within the service area; and participates in on-call schedule.

Shift: Monday-Friday 8:30am-5:30pm

Responsibilities

  • Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice
  • Adheres to all appropriate standards of accreditation and participates in quality assurance activities
  • Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution
  • Reviews, receives and verifies all new patient orders and documents on the respective order form
  • Assists the physician and other involved healthcare professionals with: Planning and implementing patient care in the home
  • Setting therapeutic objectives for drug therapy
  • Establishing parameters for monitoring the safety and efficacy of drug therapy
  • Preparing regular progress reports
  • Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
  • Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences.
  • Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules
  • Supervisory Responsibility: No

Qualifications

EDUCATION/EXPERIENCE• Bachelor of Science in Pharmacy• Doctor of Pharmacy preferred• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy• Experience with compounding infused drugs

LICENSE/CERTIFICATION/OTHER SPECIAL REQUIREMENTS• Registered Pharmacist License in state(s) of practice• Willing to obtain and maintain compliance with additional state licensure outside of home state, to support company operation

KNOWLEDGE/SKILLS/ABILITIES• Good organizational and time-management skills• Participation and membership in professional societies and organizations is desirable• Excellent verbal and written communication skills• Schedule flexibility

TRAVEL REQUIREMENTSPercentage of Travel: 0-25%

**To perform this role will require frequently sitting, standing, walking and typing on a keyboard with fingers, and occasionally bending, reaching, and climbing (stairs/ladders). The physical requirements will be the ability to push/pull and lift/carry 1-10 lbs**
