Job Description

Work Schedule: Monday to Friday, daytime business hours Anticipated Pay Range: $28 - $30 per hour This is an on-site position with occasional local travel.
As a Clinical Quality Nurse, you will play a key role in ensuring every nurse is prepared and aligned with each client's unique needs before in-home care begins. This is a great opportunity for a licensed Registered Nurse (RN) with a passion for clinical quality to make a meaningful impact. In this role, you will:

  • Conduct comprehensive pre-assignment clinical reviews with field nurses to assess readiness and appropriateness for assigned patient care.
  • Evaluate nurse qualifications, competencies, and current standing to ensure alignment with patient acuity and care requirements.
  • Review patient-specific clinical information, including medical history, plan of care, medications, and skilled needs, to ensure accurate understanding prior to care delivery.
  • Validate that assigned nurses demonstrate competency and preparedness to safely and effectively implement the physician-ordered plan of care.
  • Identify and address potential clinical risks, gaps in knowledge, or mismatches between patient needs and nurse skillset, escalating concerns as appropriate.
  • Provide clinical guidance and clarification to nurses to support safe, compliant, and high-quality care delivery.
  • Ensure compliance with regulatory, payer, and organizational standards related to patient care and staff competency.
  • Accurately document all clinical reviews, findings, and determinations in the electronic medical record (EMR) in a timely manner.
  • Collaborate with clinical leadership, case management, and scheduling teams to support appropriate patient-staff matching and continuity of care.
  • Contribute to quality improvement initiatives by identifying trends, risks, and opportunities to enhance clinical outcomes and processes.

Qualifications

  • Diploma from an accredited school or college of nursing
  • Active LVN license
  • Proficiency with electronic medical records and web-based tools
  • Strong communication, collaboration, and customer service skills
  • Excellent organizational and time management abilities
