Job Description:
This position serves as a Clinical Programs Coordinator II and is responsible for supporting multidisciplinary care coordination, facilitating patient transitions, coordinating program activities, and ensuring compliance with quality improvement and programmatic requirements. The position will coordinate services across providers, patients, families, and community resources to enhance continuity of care, support effective workflows, and optimize patient outcomes.
Coordinate care activities among pediatric nephrology providers and specialists. Facilitate communication between patients, families, and the healthcare team to promote timely access to services. Coordinate referrals, outpatient services, follow-up needs, and transition planning activities between pediatric and adult providers. Support continuity of care through effective coordination of clinical services across healthcare settings. Assist patients and families with navigating insurance requirements, authorization processes, and healthcare access barriers. Coordinate resources needed to support access to medications, procedures, and specialty care services. Connect families with financial assistance programs, community resources, and support services as appropriate. Identify and address barriers that may impact successful participation in the patient’s care plan.
Coordinate patient and family support services for pediatric nephrology patients with chronic kidney disease and complex healthcare needs. Assess barriers impacting access to care, treatment adherence, follow-up, and successful transition to adult nephrology services. Facilitate connections to appropriate clinical, behavioral health, and community resources to support patient care plans. Coordinate support services to address the medical, psychosocial, and logistical needs of patients and families living with chronic kidney disease.
Coordinate and reinforce patient and family education related to disease management, treatment expectations, and transition readiness. Support patient engagement and self-management by coordinating educational resources and reinforcing individualized care plans. Participate in Pediatric Nephrology quality improvement activities, including monitoring program goals, supporting programmatic requirements, and contributing to process improvement initiatives.
Expected Salary:
$49,000-$55,000; commensurate with education and experience.
