The Adult Bleeding & Red Cell Disorders Program of BC/Yukon is specialized provincial programs that provide interdisciplinary care, coordination, education, and support for adults living with inherited bleeding and blood disorders across British Columbia and Yukon. These programs work collaboratively with patients, families, health authorities, community partners, and specialized providers to improve access to timely, high-quality care through in-person, outreach, and virtual services. The Clinical Program Coordinator supports program operations, quality improvement, patient access, partnership development, and ongoing evaluation to strengthen care delivery across the province.

Within the context of a value-based patient, client, and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.Reports to the Patient Care Manager (PCM), the Clinical Program Coordinator:

• provides leadership in the development and implementation of optimal program processes and procedures that promote cultural diversity, equity and respect of all patients and ensure that all patients in the province of are linked to the appropriate Program of BC/Yukon
• determines patient priority and access needs and works with clerical staff to ensure optimal scheduling for all patients, including appointments for regular or episodic clinic care, in-person, outreach, or telehealth (zoom/phone) appointments as appropriate
• provides regular data reports to the team, facilitates capture of patient reported or provider reported clinical outcomes using the most appropriate IT interface (i.e., Thrive Health)
• helps prepare summaries as needed
• position works with the interdisciplinary care team to implement and evaluate changes to increase efficiency of, and access to, high quality patient services to deliver health outcomes that matter to patients. In collaboration with the PCM, works with the interdisciplinary care team to share resources across programs that optimize the opportunity for patients to access care based on their needs. Works with team on ongoing program development around case management, education, tracking processes and measuring outcome
• works with the PCM to ensure the provincial program services comply with clinical, corporate, provincial, and federal standards, processes and reporting requirements. Establishes and nurtures strategic partnership with external organizations, industry partners and key opinion leaders to enhance the program’s visibility, collaboration opportunities and access to resources. Explores and integrates cutting-edge technologies and digital health solutions to optimize patient care delivery, enhance data analytics, and improve overall program efficiency

What You Bring

Education:

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives, or in Residential Care and Mental Health, current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.

Your day to day:

• Assumes a leadership role in the development of patient advocacy programs and support services, ensuring a patient-centred approach and addressing the individualized and shared needs of individuals with chronic and/or inherited diseases. Collaborates with the interdisciplinary care team in prioritizing and scheduling appropriate and timely patient access to the applicable clinics, outreach, and related services.
• Provides case management for patients, such as for the program participants when transitioning from hospital to community, the transition of patients onto new and innovative treatments, and other situations where case management would benefit patient care.
• Develops, implements, and updates processes/criteria that make the program more effective and efficient in providing quality services to patients. Facilitates the implementation of care processes through coordination of team improvement projects and achievement of related targets/times and objectives. Identifies, leads, and supports quality improvement initiatives.
• Organizes and schedules services at appropriate pre-specified intervals, such as clinics and outreach or telehealth, based on individual patient and program needs. Ensures patients in the province are linked to the clinic, registered in the applicable electronic health record, scheduled appropriately for available services and investigations that were ordered are collated within the patients’ electronic health records.
• Collaborates with clinicians at BC Children’s Hospital and the PHC Transition Navigator to ensure that newly transitioned patients are booked at increased frequency to establish connection with the adult program and rapport with providers.
• Efficiently manages data within applicable electronic health records and online platforms as a data repository, ensures all services provided are reported to the appropriate authority. Ensures optimal use of Cerner, iCHIP, PSLS and other IT systems (i.e., Thrive Health) clinic reports and works with teams to acquire and analyze information to identify service performance and patient outcomes.
• Collaborates with the physician leads and the team on ongoing program development with respect to patient services delivery. Develops and implements short to long-term strategic plans for the provincial programs to ensure alignment with organizational goals and healthcare needs/trends.
• Monitors expenditures, identifies financial issues, investigate variances and discrepancies, and communicates budget status to PCM and applicable team. Maintains accurate and current accounting of all accounts connected to the applicable program. Manages the Clinical Consultative Service schedule and compensation submissions. Seeks opportunities to reduce costs and prepares rationale and justification for budget request.
• Integrates the Provincial Program with other internal and external/community programs and services to ensure the responsible use of fiscal and human resources, avoid duplication and complement existing resources.
• Develops and executes education programs to raise awareness about diseases/disorders, promote preventive care, and enhance engagement at a local (i.e., PHC) and provincial (i.e., Health Authorities, Community Outreach) levels.
