The Clinical Pharmacy Specialistisresponsible forproviding comprehensive pharmaceutical care to patients through clinical services, formulary management, development of policies and procedures, and integrated drug distribution. Individual willbe responsible forproviding comprehensive medication management throughclinical services,patientand provider education, andassistingwith ensuring safe medication use on the patients' transition of care (i.e., discharge) from the hospital.Individualmay alsobe responsible forproviding recommendations and/or assuming responsibility for patient care activities such as pharmacokinetics, anticoagulation, parenteral nutrition support, or other medication regimensas prescribed byprovider.

Core Responsibilities and Essential Functions:


Provide recommendations and/or manage therapy associatedwith medications, anticoagulation, antimicrobial stewardship/pharmacokinetics, parenteralnutritionor other medication regimensas prescribed bythe provider.
a. Document clinical interventions on an ongoing basis.
b. Round with interdisciplinary teams.
c. Clarify medications upon admission and discharge, as needed.
d. Optimize medication selection to improve medication access post-discharge.
e.Demonstratea basic understanding of distributive responsibilities including computer functions.
f. Assist in investigational drug studies when needed


Educational responsibilities
a. Counsel patients on their discharge medications/perform MTM services.
b. Develop and provide educational programs/in-servicesfor health care practitioners and patients as required.
c. Provide drug information when needed or requested.
d. Serve as a preceptor/instructor for students from accredited colleges/schools of pharmacy.
e. Serve as a preceptor for the Pharmacy Residency Program (where applicable).
f. Assist in developing drug protocols.


Participate on assigned Committees
a. Participate as a member of theWellstarPharmacy and Therapeutics (P&T) Committee, as well as other System and facility-specific committees as assigned or asdeemednecessary.
b.Identifytarget medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE) Tabulateand present completed MUE data to the P&T committee.
c.Monitor metrics anddevelop improvementstrategiesas necessary.
d. Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable.
e. Assist inpreparing forandmaintainingcompliance with regulatory agency (e.g., The Joint Commission) standards/requirements.
Performs other duties as assigned
Complies with all Wellstar Health System policies, standards of work, and code of conduct.


Required Minimum Education:

  • Bachelors Pharmacy or Doctorate Pharmacy-Preferred


Required Minimum License(s) and Certification(s):


All certifications arerequiredupon hire unless otherwisestated

  • RPH - Reg Pharmacist

Additional License(s) and Certification(s):


Board Certification (e.g., BCPS, BCCP, etc.), strongly preferred

Preferred Pharmacist Registration in the state of Georgia within 30 Days Required


Required Minimum Experience:


Minimum 3 years clinical pharmacy experience or PGY1 Pharmacy Residency completion Required andor
Minimum 2 years clinical pharmacy experience ina specializedareaor completion of a PGY2 Residency program Preferred
Experience with nutrition support, anticoagulation management, and pharmacokinetics Preferred


Required Minimum Skills:


Strong verbal and written communication skills High
Strong computer skills. Experience with Epic, Microsoft Office, preferred High
Capable of preparing and providing presentations High
Strong organizational skills High
