This unique position will provide dedicated clinical pharmacy support across multiple adult intensive care units, ensuring continuity of specialized pharmacy services during planned and unplanned absences, conference attendance, educational activities, and other coverage needs.The ICU Float Clinical Pharmacy Specialist will serve as a critical care pharmacotherapy expert and collaborate with multidisciplinary teams throughout UH Cleveland Medical Center's intensive care units, including the Medical ICU, Cardiac ICU, Cardiothoracic ICU, Neuro ICU, Trauma ICU, Surgical ICU, and Heart Failure ICU. This specialist will help optimize medication therapy, improve patient outcomes, advance quality initiatives, and reduce the need for cross-coverage among existing ICU Clinical Pharmacy Specialists. When not floating this Clinical Pharmacy Specialist will be based with the Cardiothoracic ICU overflow team.Position SummaryThis ICU Float Clinical Pharmacy Specialist will:· Provide dedicated clinical pharmacy coverage for adult intensive care units across UH Cleveland Medical Center based on departmental and patient care needs.Participate in multidisciplinary rounds with critical care teams and contribute to individualized patient care plans.Serve as a critical care pharmacotherapy resource for physicians, advanced practice providers, nursing staff, and pharmacists.Optimize medication therapy through evidence-based recommendations related to critical care pharmacotherapy.Provide therapeutic drug monitoring and medication management for high-risk and complex therapies.Support medication safety, quality improvement, and resource stewardship initiatives across ICU service lines.Collaborate with ICU Clinical Pharmacy Specialists to maintain consistency in practice standards, protocols, and clinical services.Support clinical teaching, research, and scholarly activities within the Department of Pharmacy. Key ResponsibilitiesParticipate in multidisciplinary patient care rounds across adult intensive care units.Provide evidence-based recommendations regarding medication selection, dosing, monitoring, and optimization for critically ill patients.Perform therapeutic drug monitoring and pharmacokinetic consultation for complex medication regimens.Assist in the management of sedation, analgesia, delirium, anticoagulation, vasoactive therapies, and other high-risk medications commonly utilized in critical care settings.Monitor for and prevent adverse drug events, medication errors, and drug-related complications.Promote appropriate utilization of formulary medications and provide guidance regarding medication-use best practices.Support the development, implementation, and maintenance of critical care guidelines, protocols, policies, and order sets.Document clinical interventions and associated outcomes.Serve as a resource and educator for pharmacists, physicians, advanced practice providers, nurses, pharmacy residents, and pharmacy students.Participate in departmental committees, quality improvement initiatives, medication-use evaluations, and research activities. EducationDoctor of Pharmacy (PharmD) required.Completion of a PGY1 Pharmacy Residency preferred.Completion of a PGY2 Critical Care Pharmacy Residency or equivalent experience strongly preferred.Work ExperienceAdvanced training or experience in critical care pharmacotherapy.Experience caring for critically ill adult patients preferred.Ability to effectively provide clinical pharmacy services across multiple ICU practice areas preferred.Previous experience with multidisciplinary critical care practice models preferred.less than 1 year Experience as a pharmacist with training in drug therapy of diseases including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients, training in cost analysis/cost evaluation techniques (Preferred)LicensureLicensed or eligible for licensure as a Registered Pharmacist (R.Ph.) in the State of Ohio required.Knowledge, Skills, and AbilitiesStrong clinical knowledge of critical care pharmacotherapy and evidence-based medication management.Expertise in therapeutic drug monitoring and medication optimization.Ability to adapt quickly to different ICU practice environments and multidisciplinary teams.Excellent communication and interdisciplinary collaboration skills.Ability to analyze complex clinical data and provide evidence-based recommendations.Demonstrated commitment to teaching, quality improvement, and advancing patient care.Computer systems skills in spreadsheet, database and word processing (Preferred proficiency) Physical Demands
