Work Shift

Day: 8 hours

To provide quality pharmaceutical antimicrobial, oncology,neonatal and pediatric, and emergency medicine management or other specialized clinical pharmacy services to inpatients, outpatients, employees and the general public. To provide education, information, and advice to physicians, nurses, and other health care professionals on the use, selection, dosage, administration of medications, and regulatory compliance . Assists the Pharmacy Director, Pharmacy Operations Manager, Pharmacy Clinical Manager and Medical Staff to coordinate and perform TJC required quality assurance activities and Drug Usage Evaluations. Coordinate and monitor infusion center chemotherapy and inpatient chemotherapy, provide antimicrobial stewardship oversight, provide neonatal and pediatric pharmacy services, provide emergency medicine pharmacy services, or other specialized clinical pharmacy services. Assists with formulary reviews as requested for Pharmacy and Therapeutics committee, assists with the coordination of staff development activities, evaluates clinical oncology, antimicrobidal stewardship, neonatal and pediatric pharmacy services and emergency medicine pharmacy services or other specialized clinical pharmacy competencies, monitors oncology, antimicrobial stewardship management, neonatal and pediatric management and emergency medicine services and/or other specialized clinical pharmacy services

Qualifications – All

Doctorate degree pharmacy (Pharm.D.) from an approved college of pharmacy.

  • One (1) year accredited pharmacy residency.
  • One (1) year experience in compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.

Assignment Specific Qualifications:

Oncology

A minimum of 2 years in an oncology practice or completion of a PGY2 Pharmacy Residency in Oncology.

Antimicrobial

A minimum of 2 years in a hospital pharmacy antimicrobial practice or completion of a PGY2 Pharmacy Residency in Infectious Disease.

Transition of Care

A minimum of 2 years in a transition of care pharmacist practice

Neonatal/Pediatric Management

A minimum of 2 years in a hospital pharmacy with neonatal and pediatric practice or completion of a PGY2 Pharmacy Residency in Pediatrics.

Emergency Medicine

A minimum of 2 years in a hospital pharmacy emergency department practice or completion of a PGY2 Pharmacy Residency in Emergency Medicine.

License/Certification/Registration Requirements

Valid California state pharmacist license with no disciplinary action(s) against the licensee that prohibits the licensee from practicing pharmacy. Valid CA driver’s license

Salary Range:

$81.23 - $121.84 USD Hour
