Title: Clinical Pharmacist
FTE: Per Diem

Per diem roles work as needed to cover scheduling needs and are expected to be available for a minimum of 3 shifts per month (leader may require greater availability). Based on department need, must be available to work holidays, weekends and off-shifts.
Provide pharmaceutical services to SPH inpatients and outpatients and to satellite clinics.
POSITION TECHNICAL RESPONSIBILITIES
Order Entry
Enter medication orders into (or perform final quality control check if performed by technical personnel) Meditech computer system and initial each order sheets to indicate pharmacist has reviewed the order(s).
Review all new orders for appropriateness (date, route, frequency, indication), duplication of therapy, safety (e.g. drug-drug interactions, drug allergy)
Resolve drug therapy problems identified by intervening with the appropriate physician or health-team member; change orders when appropriate.
Clarify all incomplete, inaccurate, or unclear orders with appropriate physician or health-team member.
Perform all necessary dosage calculations.
Calculate and pursue dosage adjustments for patients with impaired renal or hepatic function.
Perform and document therapeutic interchanges or formulary substitution on appropriate agents.
Document clinical interventions in computer system.
Patient Care
Review all patients’ computerized medication profiles-intervene if a drug therapy problem is identified.
Provide drug information for requests immediately affecting patient care.
Perform drug monitoring consults (i.e. pharmacokinetic consultations).
Manage TPN orders-review and document labs and perform necessary interventions.
Perform therapeutic drug monitoring on appropriate drugs (review and assess appropriate labs and drug levels; e.g. warfarin, anticonvulsants, digoxin).
Review all patients’ labs and microbiology cultures and intervene with appropriate physician if necessary.
Patient related service documentation (e.g. performing and documenting 1st dose teaching, diabetes teaching, warfarin reaching, discharge counseling).
Other
Guide and perform final quality control check of duties that may be performed by technical personnel: drug selection, preparation, packaging, labeling, etc. including unit dose cart check.
Prepare or supervise/check the preparation of parental sterile products; including injections, IV admixtures, TPN.
Instruct on the proper methods of administering drugs and precautions/side effects involved in their use.
Insure timely delivery of medications to all patient populations served.
Assist with the training of new department employees.
Provide drug information and pharmacotherapy recommendations to other healthcare professionals.
Maintain proper drug storage, record keeping, and utilization data as required by state and federal regulations and good practice standards.
Maintain contemporary working knowledge of pertinent state & federal laws and regulation and hospital and pharmacy department policies and procedures that govern contemporary hospital pharmacy practice.
Perform inspection of drug stocks at designated storage areas to verify proper storage conditions, expiration dates, quantities, security, etc.
Read selectively from professional literature, local policies/procedures and professional practice standards to stay abreast of new drug entities, precautions involving drugs, drug shortages and recalls, and legal requirements of pharmacy practice.
Perform as Department of Pharmacy member of the hospital “Code Blue” cardiopulmonary resuscitation team.
Document and report adverse drug reactions.
Presence and participation in designated hospital committees.
Provide direct supervision of pharmacy technicians (1-3) and one intern/student at any time.
Provide community service presentations, drug information, and prevention programs to consumer groups and patient groups.

Education:
Required: PharmD degree or Bachelor’s degree in Pharmacy with equivalent experience.

Experience:
Required: 1-2 years of hospital experience as a Pharmacist.

Licenses and Registrations:
Required: State of Wisconsin license as a Pharmacist

Certification(s):
Required: Basic Life Support (BLS) certification within 3 months of hire and Advanced Cardia Life Support (ACLS) within 6 months of hire.
