The Clinical Pharmacist – Inpatient provides comprehensive clinical and operational pharmacy services to support safe and effective medication therapy for hospitalized patients. Responsibilities include medication order verification and clarification, drug therapy evaluation and dosing optimization, monitoring for interactions and therapeutic appropriateness, providing pharmacokinetic consultations, supporting sterile product preparation, and participating in interdisciplinary rounds when scheduled in a clinical capacity. This caregiver also provides operational pharmacy coverage as needed while ensuring medication safety and regulatory compliance.
This role has the opportunity to cross-train in other specialties such as ED, critical care, etc.
A caregiver in this position will work rotating shifts with three weeks on first shift from approximately 8:00am - 4:30pm, followed by one week on second shift from approximately 1:00pm - 9:30pm. Caregivers participate in an every fourth weekend rotation, working Saturday and Sunday, and are expected to work 1–2 holidays per year, with no more than two holidays. No float expectations are associated with this role.
A caregiver who excels in this role will:
Provide safe, efficient, patient-centered, evidence-based medication services.
Verify medication orders and/or dispense medications accurately according to state and federal regulations.
Document services in a complete, accurate and timely manner.
Ensure all pharmacy operations comply with national accreditation standards.
Maintain professional interactions with patients to facilitate successful patient experiences.
Educate and train students, residents, other caregivers, and/or patients.
Minimum qualifications for the ideal future caregiver include:
PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy
Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days (for individuals required to administer immunizations).
Current Advanced Cardiovascular Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association (AHA)or American Red Cross. For caregivers required to respond to adult or pediatric cardiopulmonary arrest, this is required within one year of hire
Preferred qualifications for the ideal future caregiver include:
Board certification
Completion of a post-graduate year 1 residency (PGY1), fellowship or 2 years of relevant post-graduate pharmacy experience
Post-graduate year 2 residency (PGY2) or relevant experience for Clinical Pharmacists hired in a specialized pharmacy practice area
