Mercy Hospital is seeking a dedicated Clinical Pharmacist to join our team. This role plays a vital part in delivering safe, effective medication therapy and serving as a trusted clinical resource for patients and the healthcare team.
The Clinical Pharmacist collaborates with physicians, nurses, and other healthcare professionals to optimize patient outcomes through evidence-based medication management. This role ensures safe medication use, provides drug information, and supports Mercy’s mission, values, and Service Standards.
Education
Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy
Licensure
Current pharmacist license in the state of practice
Review patient records to assess appropriateness of medication therapy
Evaluate patient conditions to ensure all health issues are properly treated
Develop and implement safe, effective medication plans
Monitor drug therapy and intervene when appropriate
Assess outcomes and adjust treatments as needed
Provide counseling to patients on proper medication use
Collaborate with physicians and nurses on medication selection, dosing, and therapy plans
Identify and prevent adverse drug events, interactions, and contraindications
Evaluate medication orders for clinical accuracy and operational considerations
Ensure compliance with formulary guidelines and care pathways
Maintain accurate documentation of patient care activities
Participate in medication use evaluations (MUE) and adverse drug reaction reporting
Perform required inspections, expiration checks, and floor stock reviews
Serve as a drug information resource for medical, nursing, and pharmacy staff
Precept pharmacy students and contribute to staff education initiatives
Present in-services and educational programs
Support competency development within the department
Participate in committees and departmental initiatives
Adhere to safety, confidentiality, and regulatory standards
Participate in quality assurance and performance improvement activities
Maintain current knowledge of policies, procedures, and clinical best practices
Complete required safety training and continuing education
Utilize pharmacy systems for order verification and clinical documentation (e.g., Epic)
Use drug information resources (e.g., Micromedex)
Operate pharmacy automation systems (Omnicell, McKesson, ScriptPro, etc.)
Demonstrate Mercy’s Signature Service communication and customer service standards
Maintain strong working relationships with interdisciplinary teams
Communicate effectively across shifts
Uphold professional appearance and conduct
Work efficiently under time-sensitive conditions
