As a Clinical Pharmacist, a caregiver in this role will serve as the primary pharmacist supporting the inpatient Hematology/Oncology service within a 256-bed hospital caring for a high-acuity patient population, including a robust bone marrow and stem cell transplant program. The caregiver will participate in multidisciplinary rounds, collaborate with providers on patient care plans and review, verify and optimize chemotherapy and supportive care medication orders in Epic. Responsibilities will include clinical medication monitoring, therapeutic interventions, medication safety and accuracy, drug information consultation and monitoring patients for medication efficacy, toxicity and adherence to treatment protocols. This role will also support oncology and transplant-related medication management and assist with operational pharmacy staffing as needed.
