Job Description

The Clinical Pharmacist is an integral part of the medical team. The incumbent participates in medical rounds, assists the multi-disciplinary team to select the most cost-effective evidence-based treatment for the patients, performs pediatric dosing based on Pharmacokinetic parameters, manages Narrow Therapeutic Index drugs by using lab values to calculate and predict desired drug levels and participates in discharge planning and medication reconciliation. S/he participates in Drug Information activities or any assigned ambulatory clinics. S/he also participates in appropriate committees and provides valuable input as well as provide teaching of medication intricacies to Medical and Pharmacy Students and residents.

  • As part of a multidisciplinary team, participates in medical rounds and assists with pharmaceutical treatment decisions.
  • Recognizes untreated health problems that could be improved or resolved with appropriate medication therapy.
  • Reviews patients’ medication orders for efficacy and safety, and makes recommendations to Prescriber if changes are required.
  • Implements drug level monitoring protocols and modifies processes to ensure optimization and accuracy of the drug monitoring in alignment with Sidra and State of Qatar regulations.
  • Provides recommendations in committees related to medication use, processes and resource optimizations.

CLINICAL

  • Assesses the status of the patient’s health problems and determines whether the prescribed medications are optimally meeting the patient’s needs and goals of care.
  • Fully integrates her/his specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, in when performing patient care activities in collaboration with other members of the health care team.
  • Utilizes professional expertise to provide patients with customized treatment strategies to best meet individual patient needs.
  • Provides medication information to healthcare professionals, including but not limited to, Prescribers and Nurses.
  • Participates in patient care rounds.
  • Checks and verifies medication reconciliation on admission and discharge.
  • Performs Drug-Drug, Drug-Lab, Drug Food and Drug-Disease Monitoring and offers alternatives when applicable.
  • Assists in the ordering of total parenteral nutrition by being part of the Total Parentral Nutrition (TPN) team, as per the needs of her/his assigned care center within hospital.
  • Completes therapeutic drug level monitoring protocols to ensure patient safety and optimal efficacy of treatment.
  • Participates in Clinical Pharmacy activities for his/her care center, including (but not limited to) Pharmacokinetics consultations, antimicrobial stewardship, drug protocol management, Drug Information Center or Ambulatory Clinics, and Medication Use Evaluation (MUE).
  • Ensures continuity of care between the acute and ambulatory patient care environments.
  • Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens.
  • Documents all clinical activities and interventions accurately and completely, as per policy and procedure.
  • Participates in projects with the local College of Pharmacy and local Pharmacy organizations as needed.

COMPETENCY

  • Provides in-service education to Pharmacy and non-Pharmacy personnel.
  • Participates in the orientation and training of Pharmacy students, residents and newly hired Pharmacists and Technicians.
  • Communicates effectively with healthcare professionals, patients and family members where appropriate.
  • Demonstrates proficiency in the compounding of extemporaneous preparations.
  • Demonstrates proficiency in aseptic technique during the preparation of sterile pharmaceutical products.
  • Demonstrates working knowledge of departmental equipment and software where applicable for assigned duties.
  • Has (or will obtain) certification in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP), and trains with code team and attends code situations, as required by assigned care center(s).
  • Maintains professional competency and skills sufficient to meet current and future needs of the facility.

MEDICATION SAFETY

  • Provides patients with thorough medication counseling, including how to best take his or her medications.
  • Monitors and reports adverse drug reactions.
  • Adheres to policies and procedures relating to Medication Safety, and provides suggestions for increasing it.
  • Supports and participates in the goals of the organization's quality improvement, risk management, and patient safety programs to include (but not limited to) National Patient Safety Goals, Core Measures, and other medication-related Joint Commission International (JCI) initiatives.

OPERATIONAL

  • Contributes to the achievement of goals of the department through personal effort and influence over others.
  • Liaises with the medical service and department of Pharmacy to resolve any issues facing the service or the Pharmacy.
  • When required, helps support verification activities in the Pharmacy.Verifies medication orders for accuracy and clinical appropriateness, checks medications prepared by Pharmacy Technicians for accuracy prior to dispensing and notifies appropriate persons if changes are required.
  • Provides accurate shift-to-shift communication regarding unsolved situations requiring future action.
  • Provides professional, courteous customer service to all customer types (patients/family, Nursing, Medical Staff, others).

QUALITY IMPROVEMENT

  • Sits on committees as required to provide recommendations pertaining to medication use and processes.
  • Participates in the Process/Quality Improvement activities of the care center, including Key Performance Indicators (KPI’s).

REGULATORY

  • Complies with accreditation, legal, regulatory, and safety requirements for a specific practice setting.
  • Completes all required Continuing Education activities to maintain licensure to practice as a Pharmacist in Qatar, per the Qatar Council for Healthcare Practitioners (QCHP).
  • Adheres to Sidra’s standards as they appear in the Code of Conduct and Conflict of Interest policies

In view of the evolving needs and opportunities within Sidra, this position may be required to perform other duties as assigned and reporting relationships may vary.

ESSENTIALPREFERREDEducationDoctor of Pharmacy (PharmD) from an ACPE / or equivalent accredited School of Pharmacy or Postgraduate Diploma in Clinical Pharmacy (PGDipClinicPharm)

Residency or fellowship in a specialty area in Pediatrics or Women’s Health

Experience

  • 3+ years’ experience* as a licensed/registered clinical Pharmacist in a hospital setting
  • Exceptions to the experience requirement may be granted for Qatari nationals (internal and external candidates), with a reduction of up to two (2) years
  • Licensed in Pharmacy in country of origin
  • Obtain State of Qatar Pharmacy License after on boarding

*Experience can be pre or post PharmD qualification

**Pharmacy internship period is not counted in the experience

Clinical pharmacy experience in a pediatric hospital settingCertification and Licensure Professional Membership Member of professional hospital pharmacy association in country of origin.Job Specific Skills and Abilities

  • Excellent clinical knowledge as it pertains to medication and disease management
  • Demonstrated ability to work with Pharmacy Information Systems
  • Proficiency with Microsoft Office suite
  • Fluency in written and spoken English
