C-Care is currently recruiting a dedicated and proactive Clinical Pharmacist, passionate about improving patient care, to join C-Care Darné.

ROLE PURPOSE

In collaboration with the medical personnel, the Clinical Pharmacist contributes to the continuous improvement of patient care. The CP realizes descriptive analysis of pharmaceutical interventions and advises, notably on drug prescription to evaluate their relevance.

Clinical Pharmacist works in close collaboration with Doctors, Nursing, Quality and Supply Chain to facilitate the continuous flow of information necessary to guarantee supply, prescription, dispense, and administration of drugs. He closely follows and reports any deviation throughout the process in order to act on them and prevent them.

DUTIES AND RESPONSIBILITIES
  • Be responsible for all drug-related quality control i.e. check of prescriptions, regular audit on drugs on medication management process, root cause investigations when incidents related to medication management are identified. Lead Quality, Audit and risk processes pertaining to medication management.
  • Coordinate regularly with Supply Chain to be informed of availability of drugs and communicate accordingly to Doctors, especially in case of shortages in an attempt to discuss which potential alternatives to acquire.
  • Be an active support to ward nurses in their management of drugs, ensuring no expired drugs are available at the ward that may be dispensed to patient.
  • Perform a monthly control of the Crash Carts available in each hospital unit. The control will include a full count of drugs, exclusion and listing of expired drugs and replenishment of the crash cart as necessary.
  • Perform a monthly control of the Dangerous Drugs available in each hospital unit. The control will include a full stock count of the DD on stock, and audit reconciliation of the DD usage over the month reviewed. Any discrepancy must be reported to the head of Nursing, and Head of Supply Chain.
  • Report any medication error or incident and perform the necessary investigation to define action plan accordingly. Be the contact person to the Health Authorities (MoH) for any pharmacovigilance-related cases that must be investigated and reported.
  • Be the contact person to investigate on Continuous Quality Improvement (CQI) open cases related to pharmacy or drugs incident. Such cases can be linked but not exclusive to prescription errors, dispensing errors, drug administration errors, drug unavailability, adverse events, drug inefficacy.
  • In the frame of the CHKS accreditation process, be the contact person for Clinical Pharmacy-related topics, responsible for collecting the data to build and maintain indicators and to work on SOP and policies requested as part of the pharmacy requirements.
  • In collaboration with the quality and training department, ensures training on SOP implementation and whenever improvement on pharmaceutical processes are necessary. Perform regular technical training of Pharmacist Assistant, Nurses, Doctors and other healthcare staff as needed.
  • Act as the Secretary of the Drug & Therapeutics Committee; take the minutes and follow-up on the action plan defined to ensure timely execution. Schedule and send invitations for the Drug & Therapeutics Committee at the frequency defined in its terms of reference. Capture and report attendance.
  • Attend and play an active role in all other clinical governance bodies necessary to the flawless execution of the Clinical operations and patient care.
  • Assist the Consultants and medical teams (MOs + Nurses) in day-to-day medication management of the patients to ensure the best clinical outcome
  • Develops and oversees policies and procedures for drug purchasing, drug usage, drug distribution, and drug control
  • Works as an active member of multidisciplinary team and attends multidisciplinary round – for Cancer as well as other specialties – Work closely with consultants of the best medication available
  • Provides POD review when requested.
  • Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assures continuity of pharmaceutical care between shifts and among staff
  • Identifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs
  • Audit and monitor how medications are being indented, prescribed, transcribed, monitored, administered and billed to the patient
  • Any cognate duties in relation to the role and as requested by the head of department.
  • Lead strategic projects like Antibiotic Stewardship, PDCA cycles & online prescription project
  • Ensure compliance with the Pharmacy Council Act 2015 and/or relevant legislations and standards of practice which govern the ethical profession of pharmacy.

Qualifications and Experience
  • Doctor in Pharmacy
  • Additional Degree in Business Administration preferred
  • Minimum of 5 years of experience as Pharmacist in hospital / Clinical environment
