Under the direction of the Pharmacy Director, the Clinical Pharmacist provides optimal therapeutic medication regimens for Acute Care, Critical Care, Oncology, long term care, and Transitions of Care for adult, pediatric and geriatric patients. The Clinical Pharmacist reviews, assesses and intervenes in patient medication therapy, processing medication orders, dispensing medications, providing drug information to health care professionals and education to patients. Oversees the application of pharmacy law with hospital/pharmacy policies and procedures on a day-to-day basis.
