Schedule: Monday–Friday with site coverage rotation between Caroline, Brunswick, and Charles City locations. This is a high-volume practice offering a rewarding, team-oriented work environment.
ESSENTIAL JOB FUNCTIONS:
Be responsible for ensuring that the practice of pharmacy is in overall compliance with laws and regulations.
Provides professional pharmacy services as appropriate.
Assists in development of appropriate clinical pharmacy goals and objectives.
Maintains and uses up-to-date patient drug profiles.
Provides patient medication counseling and education through the proper use of labels, accessory labels and personal contact.
Implements and provides all aspects of clinical pharmacy activities with emphasis on therapeutic drug monitoring, clinical pathways, formulary management, Pharmacoeconomics and patient outcomes through detailed reporting systems.
Implements policies, practices and procedures for clinical pharmacy services, ensuring compliance with regulatory standards.
Performs Medicare Annual Wellness Visits which will include, but is not limited to, medication reconciliation, laboratory reviews, medication appropriateness, immunizations required, preventative health screenings required, and address any questions that the patient may have about their care.
Collaborates with physicians in establishing drug criteria for the safe, economical and appropriate use of medications.
Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and healthcare providers including therapeutic duplications, medication allergies, clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.
Administer immunizations based on collaborative practice agreement or standing orders.
Performs initial assessments of drugs having pre-established criteria to be monitored and monitors these drugs for follow-ups.
Participate in Medication Therapy Management and Comprehensive Medication Reviews as applicable.
Retrieves, analyzes, evaluates, and interprets the scientific literature, and serves as the resource person for patient and population-specific drug information to health professionals and patients.
Participates in the generation of new knowledge relevant to the practice of pharmacotherapy, clinical pharmacy and medicine.
Educates health care professionals, pharmacy students, patients, and the public regarding rational drug therapy.
Interprets and applies pharmacokinetic drug data, and recommends dosing regimens based on patient's medical history and disease state, ensuring optimal patient care.
Establishes and implements programs that help ensure cost-effective drug therapy and successful patient outcomes.
Attends conferences, seminars, and meetings as required.
Uses computer applications or other automated systems such as spreadsheets, word processing, calendar, e-mail and database software in performing work assignments.
Be familiar with and follows the policy and procedures of CVHS and CVHS Pharmacy.
Accesses protected health information (PHI) in accordance with departmental assignments and guidelines.
All CVHS Pharmacy staff will abide by the standards of accreditation in Mail Order Pharmacy and Specialty Pharmacy. 
May perform other incidental tasks, as needed.
Attends work as scheduled.
KNOWLEDGE, SKILLS, AND ABILITIES:
Demonstrates competence in performing essential job functions.
Demonstrates interest in assisting the Director of Pharmacy Services in the overall operations of the Pharmacy Department.
Sensitivity to the personal feelings and needs of patients and employees.
A desire to interact with the professional staff in promoting a clinical pharmacy program.
Well-developed communications skills and to maintain a positive, professional appearance and attitude in dealing with staff and patients.

QualificationsEDUCATION, TRAINING AND EXPERIENCE:
Graduation from an accredited Pharmacy School with a Bachelor of Science in Pharmacy or PharmD.
Licensed to practice pharmacy in the Commonwealth of Virginia.
License is free of restrictions to bill Medicaid and Medicare.
Completion of pharmacy-based immunization certificate or equivalent.
Basic Life Support CPR certification.

In November 1970, Central Virginia Health Services opened in rural Buckingham County, becoming the first community health center in Virginia. The site, Central Virginia Community Health Center (CVCHC) was created to serve the health care needs of residents of the rural counties of Buckingham, Cumberland, and Fluvanna. Initially operating out of three trailers in each of the three counties, practice operations were consolidated into the Gold Hill community of Buckingham County.

CVHS has grown and today is Virginia’s largest community health center both in terms of numbers of sites as well as number of patients. Each site added to CVHS was developed as a result of identified needs in each respective community. Over time, it became apparent that urban as well as rural communities struggled to meet residents’ health care needs.
