Shift:

The patient navigator will work with the whole-patient care through intensive case management. Patient Navigator will work one-on-one with clients to encourage continued commitment and adherence to medical treatment, access to social services, improved communication, and prompt re-engagement in care.

Purpose:

What you will do:

* Plans and implements programs to meet the needs of the Community Health and Well-Being department.

* Collaborates with physicians/APRNs, hospital staff, financial services on developing policies and procedures for the care of the patient.

* Facilitates patient in applying for financial assistance.

* Facilitates linkages to most appropriate site of care to ensure necessary treatment/intervention/care.

* Coordinates and facilitates treatment objectives for the patient.

* Develops informational and educational materials for this patient population.

* Assists in meeting the informational needs of each patient s primary medical staff and colleagues of Holy Cross Hospital and HealthPlex and acts as a resource for both.

* Serves as a liaison to services and resources within the hospital and the community.

* Responds to and provides community education presentations.

* Proactively educates community-based medical providers of services available.

* May serve as a facilitator of selected support groups.

* Provides assistance to patient and family to fill out appropriate financial eligibility applications and filing for financial assistance. Works with the Holy Cross and System Office Financial Assistance program.

* Develops and maintains a written plan of intervention for patients with a new cancer diagnosis.

* Works collaboratively with physicians/APRNs and the oncology care team members.

* Communicates patient care plans to primary care physicians/APRNs and obtains consensus for participation in this service.

* Advocates for the use of the Bienes Cancer Center services in the next phase treatment plan and provides linkage of those services.

* Provides emotional support and education to the patient (and family) during each phase of treatment up to and including follow up treatments and monitoring.

**RNs with oncology experience not required but preferred for this role

Additional job description requirements

* Tracks and coordinates care for patients with new malignant diagnosis.

* Tracks time from diagnosis to multidisciplinary evaluation by medical oncology and surgical oncology.

* Develops relationship with American Cancer Society, South Florida Cancer Care,

Florida Breast Foundation, Komen Foundation, Gilda’s Club and other cancer related support and funding services and other patient support groups.

* Participates in Cancer Center and Breast Cancer quarterly meetings and provides CHWB outreach report inclusive of educational events and statistics.

* Participates in multi-disciplinary oncology patient conferences when patient cases are being reviewed.

* Tracks compliance with the NCCN guidelines, treatment and surveillance.

* Respects and maintains confidentiality of patient, staff and privileged hospital information.
