Clinical Nurse Manager - Home Health - 100% Days - HSSB Building-(26003543)

Provides leadership and oversight for home health patient care services, including coordination of care across the continuum from inpatient admission through discharge and transition to home-based services. Responsible for planning, organizing, directing, evaluating, and improving clinical operations, staff performance, quality outcomes, and patient satisfaction within the home health program. Functions as a clinical nurse expert, ensuring delivery of high-quality, compliant, and patient-centered care in accordance with regulatory standards, accreditation requirements, and organizational goals.

Position Responsibilities:

Human Resource Management

  • Hire, develop, and manage the performance of Home Health staff.

  • Assist with orientation and onboarding of new personnel.

  • Assess competencies and support ongoing staff development.

  • Develop succession planning strategies and promote staff engagement.

  • Ensure appropriate staffing coverage, including evenings, nights and weekends.

Financial Management

  • Prepare budget or assist in development of departmental budgets and financial projections.

  • Monitor and manage home health program financial performance, including awareness of Medicare and third-party payer requirements.

  • Analyze variances and create action plans to take corrective action.

Operations, Standards of Care, Regulatory Compliance, Quality and Patient Satisfaction

  • Review patient satisfaction data and development, implement, and evaluate plan(s) to enhance patient satisfaction.

  • Ensure compliance with all applicable Home Health regulations, accreditation standards, and organizational policies.

  • Maintain survey readiness and regulatory compliance.

  • Participate in quality improvement initiatives and monitor patient outcomes and satisfaction data.

  • Maintain integrity and timeliness of clinical documentation.

  • Develop and implement policies and procedures for home health services.

  • Provide leadership coverage in absence of senior home health leadership as needed.

Patient Care/Services Delivery

  • Assess care coordination within the multidisciplinary team to maximize patient satisfaction and continuity of care.

  • Identify needs, trends, and forecast to provide quality patient care and ensure appropriate flex staffing.

  • Ensure individualized plans of care are developed, implemented, and evaluated for home health patients.

  • Support occasional direct patient care activities as needed.

Planning & Goal Management

  • Develop action plans for Home Health that meet healthcare strategic goals.

  • Monitor/evaluate goal implementation and provide progress reports to nursing leadership.

Communication & Collaboration

  • Advocate for the needs of the unit/clinic with healthcare leaders.

  • Facilitate multidisciplinary participation in clinical problems-solving.

  • Utilize unit councils to identify opportunities for improvement and innovative models of care using a multidisciplinary team to impact patient outcomes.

  • Collaborate with physicians, interdisciplinary teams, and community partners to support patient care delivery.

  • Serve on Home Health committees and facilitate meetings or case conferences as needed.

Education Programs

  • Educate and mentor staff in the management of complex patient care issues.

  • Provide clinical education and coaching to develop staff competency and professionalism.

  • Participate in and support the education of healthcare students.

Research administration

  • Encourage staff to participate in evidence-based practice research initiatives and incorporate best practices into unit operations.

Percent of Time: 100%
Schedule 40 hours per week, 8:00 - 4:30

Qualifications

Education Requirements:

  • Masters degree in nursing or related field. If master's degree is in a related field, a Baccalaureate Degree in nursing is required.
  • Must obtain an advanced degree in nursing, health care administration or business administration within (5) five years of beginning this position.
  • Current license to practice nursing in Iowa or ability to obtain an Iowa license upon hire.

Experience Requirements:

  • Considerable (3-5 years) experience in leadership/ management roles including clinical experience and acute care.

  • Experience with unit operations including human resources, financial management, and compliance with regulatory environment with various federal and state agencies and the Joint Commission.

  • Must be proficient in computer software applications.

  • Professional experience working effectively with individuals from a variety of backgrounds and perspectives.

  • Demonstrates excellent written and verbal communication skills.
