We support people from all walks of life through specialist mental health programs, community nursing, and NDIS services.

The Opportunity

We are seeking a Clinical Nurse Consultant (CNC) to provide advanced clinical leadership and governance across Fortify Care Services. Reporting to the Head of Care, you will oversee Nurse Case Managers and Clinical Nurse Educators, ensuring care is delivered safely, compliantly, and with measurable impact.

This role combines specialist nursing expertise with operational oversight, guiding teams to deliver evidence-based, person-centred care while strengthening quality, risk management, and continuous improvement.

Key Responsibilities

  • Monitor and contribute to clinical policies, procedures, and guidelines to ensure best practice and compliance with NDIS and regulatory requirements.
  • Supervise and mentor Nurse Case Managers and Clinical Nurse Educators, fostering collaboration with interdisciplinary teams.
  • Advise on and support improvement initiatives, monitoring and evaluating nursing care practices and service delivery outcomes.
  • Lead or delegate internal clinical audits, identifying risks and areas for improvement.
  • Hold a small caseload of participants, providing direct clinical case management and modelling best practice.
  • Act as a senior clinical contact for participants, families, and external stakeholders when complex or sensitive issues arise.

Selection Criteria

Skills & Experience

  • At least 6 years’ clinical experience in disability services or a related healthcare setting, including leadership or consultancy roles.
  • Strong clinical assessment and problem-solving skills, with the ability to manage complex situations.
  • Excellent communication, interpersonal, and leadership abilities.
  • Commitment to person-centred care, ethical practice, and cultural competence.
  • Proficiency in computer skills and documentation.

Qualifications & Other Requirements

  • Bachelor of Nursing degree.
  • Current registration as a Registered Nurse.
  • Postgraduate qualification in a relevant clinical specialty (e.g., disability nursing, rehabilitation nursing) highly desirable.
  • Health Service Management and training/assessment qualifications highly desirable.
  • Current Queensland driver’s license.
  • NDIS worker screening check (or ability to obtain)
  • Rights to work in Australia.

What We Offer

  • A supportive, values-driven team environment.
  • Opportunities for professional development and career growth.
  • The chance to make a meaningful difference in people’s lives.
