The Opportunity

As a Clinical Nurse Case Manager, you will lead complex, person‑centred clinical case management within the NDIS framework. You’ll coordinate multidisciplinary input, develop and review care plans, and ensure high‑quality, evidence‑based care that aligns with Fortify’s clinical standards and Positive Behaviour Support Plans (PBSPs) where applicable. The role blends hands‑on clinical leadership with a capacity‑building focus, mentoring EN and disability support workers in the field to deliver safe, consistent supports across community settings.

Key Responsibilities

  • Clinical Case Management: Hold a defined caseload; complete comprehensive assessments; develop, implement, and review care plans in collaboration with participants, families, and multidisciplinary teams; ensure alignment with NDIS plans and PBSPs.
  • Clinical Leadership & Escalation: Provide advanced clinical decision‑making within RN scope; escalate complex matters through the CNC escalation process when required.
  • Capacity Building & Mentoring: Coach EN and DSW frontline staff; deliver in‑the‑moment coaching and scheduled training that reflects written care and PBSP guidelines.
  • Quality Assurance & Governance: Uphold Fortify policies and care standards; maintain accurate, timely documentation; participate in audits and continuous improvement activities.
  • Coordination & Stakeholder Engagement: Liaise with Operations/Rostering and external partners to ensure seamless intakes, service coordination, and risk‑aware staffing that matches individual support needs.
  • Risk Management: Identify clinical risks and early deterioration; initiate mitigation plans and escalate via CNC/HOC pathways for urgent review.
  • Participate in a clinical on-call roster to provide advice and guidance to staff.
  • On‑the‑Floor Support: Work a rostered pattern across office‑based tasks and community‑based coaching/training; provide direct support and shift coverage to maintain continuity of care when needed.

Selection Criteria

Skills & Experience

  • At least 4 years’ clinical experience in disability services or a related healthcare setting, including leadership or consultancy roles.
  • Strong clinical assessment and problem-solving skills, with the ability to manage complex situations.
  • Excellent communication, interpersonal, and leadership abilities.
  • Commitment to person-centred care, ethical practice, and cultural competence.
  • Proficiency in computer skills and documentation.

Qualifications & Other Requirements

  • Bachelor of Nursing degree.
  • Current registration as a Registered Nurse.
  • Postgraduate qualification in a relevant clinical specialty (e.g., disability nursing, rehabilitation nursing) highly desirable.
  • Health Service Management and training/assessment qualifications highly desirable.
  • Current Queensland driver’s license.
  • NDIS worker screening check (or ability to obtain)
  • Rights to work in Australia.
