About the role

Mater has an exciting opportunity for an experienced Geriatric Nurse Care Coordinator (CN) to work within the Hospital in the Home (HITH) Nursing Team. They will act as the central point of contact between patients, families, doctors, and community service providers to ensure continuous, person-centred care, reduce hospital admissions, and help seniors maintain independence in their homes. This role is offered as a permanent role working .2 FTE or 2 x 7:36 (hr:mm) shifts per fortnight.

The Hospital in the Home (HITH) Geriatric Nurse Care Coordinator is a dynamic and self-directed role that will manage the comprehensive care needs of older adults, particularly those with complex, chronic, or frail health conditions. They will work within the HITH team, which delivers hospital-level care in the safety and comfort of a person’s residence. Using extensive experience, this role will provide comprehensive coordinated care to these patients and work with the existing HITH team to review protocols and support the development of their peers in the geriatric space.

Salary range: $54.44 - $58.30 plus superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!

What you’ll be doing

Key responsibilities for this role include:

• Achieving optimal patient outcomes throughout the patient journey and continuum of care
• Completing comprehensive assessments in clients’ homes or care settings to design, implement, and regularly review individualised care plans that reflect changing needs.
• Serving as the central point of contact, coordinating care across GPs, specialists, allied health providers, and community services to ensure seamless, integrated support.
• Providing ongoing clinical monitoring of chronic and complex conditions such as dementia, diabetes, and arthritis, including medication management and timely clinical interventions.
• Partnering with clients and their families to explain care plans, support disease management, and guide them through the aged care system with confidence.
• Proactively managing transitions between hospital, rehabilitation, and home to promote continuity of care and reduce avoidable hospital admissions.
• Ensuring all care delivery meets Aged Care Quality Standards, maintaining accurate clinical documentation, and supporting effective funding utilisation (including AN-ACC).
• Providing clinical guidance and mentorship to care teams while advocating for older people’s preferences, dignity, and right to informed choice
• Proactively identifying and supporting continuous quality improvement and risk management strategies, ensuring integrated and responsive HITH service provision
• Maintaining currency of clinical knowledge and skills and achievement of new clinical competencies as required to deliver new treatment protocols as the service expands
• Commitment to fulfil professional development objectives as determined in the professional development plan
• Maintenance of corporate and clinical mandatory training in line with the service’s key performance indicators
• Commitment to impart knowledge and share skills through supervision of nursing students and mentorship of less experienced team members
• Exceptional time management and self-management skills as required for autonomous workload management, including the ability to self-identify fatigue and observe strategies to ensure driver safety
• Excellent stewardship of Mater resources, maximising patient access to nursing services, use of consumables and fleet management, and human resources
• Demonstrate high-level problem-solving skills and the ability to identify concerns requiring escalation

About you

To be successful in your application, you will have:

• Current AHPRA registration as a Registered Nurse (no restrictions)
• Minimum 2 years clinical experience in aged care, rehabilitation or HITH.
• Current open driver’s licence
• Postgraduate qualifications in a related discipline is desirable but not essential
• Previous experience as a liaison officer, discharge coordinator or navigator position is highly desirable
• Excellent written and verbal communication skill and the ability to liaise with a variety of different stakeholders both internally and externally at various levels
• Demonstrated ability to operate in a team environment and autonomously using initiative, sound reasoning and strong listening, negotiation, and decision-making skills
• Strong attention to detail and organisational skills
• Commitment to ensuring a responsive and streamlined GEMHITH service is always delivered to patients.
