Clinical Manager Specialist

The Clinical Manager Specialist (CMS) is a registered professional nurse who is able to direct all clinical activities for a hospice location on an interim basis during periods of high patient census or Patient Care Manger vacancies. Works closely with the Executive Director, Area Clinical Manager and Regional Vice President of Clinical Operations to ensure that the quality of patient care and efficiency of clinical operations meet Heart to Heart expectations. The CMS participates in the interviewing and mentoring of replacement Patient Care Managers if necessary.

QUALIFICATIONS AND EDUCATION REQUIREMENTS
• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State license as a Registered Nurse
• Current State Driver’s License
• Two years of experience as a Registered Nurse in a clinical care setting, hospice or home health preferred
• Demonstrated ability to supervise
• Strong critical thinking, organizational, interpersonal, and communication skills
• Experience in EMR a plus
• Specialized skills preferred in palliation and end-of-life
• Must meet mandatory CE requirements of licensing board per state and maintain a current status
• Must have reliable transportation and provide ongoing valid and current auto liability insurance

ESSENTIAL DUTIES:
• Direct and coordinate clinical departments and sites; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations (Conditions of Participation)
• Supervise and provide direction to nursing staff and team members, in an effort to ensure quality, compliance with Plan of Care, assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel
• Supports employee and patient education to enhance knowledge, skills and achieve quality experiences. Continuously trains nursing staff to retain employees with the highest quality of patient care
• Monitor QA/PI Program and assures appropriate corrective measures are performed
• Promote and educate regarding the concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Employ qualified personnel and ensure adequate staff education and evaluation, including orientation, in-service, continuing education, competency testing and performance improvement
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
