At Home Care of Kentucky is a growing, family-centered home care agency dedicated to delivering high-quality private duty nursing services across Kentucky. We partner closely with hospitals, case managers, families, and payers to ensure seamless care transitions and exceptional outcomes.
As our Private Duty Nursing program continues to grow, we are seeking a Clinical Manager to lead and support our clinical operations and admissions process.

The Clinical Manager – Private Duty Nursing plays a key leadership role in overseeing clinical intake, authorizations, admissions, and ongoing case management. This position serves as the clinical bridge between hospitals, families, nurses, and payers—ensuring safe, timely, and compliant care delivery.
Key Responsibilities
Clinical Intake & Admissions
• Review and manage referrals for private duty nursing services
• Coordinate hospital discharges and start-of-care admissions
• Organize and oversee new patient onboarding and clinical documentation
Insurance & Authorization Management
• Coordinate and manage insurance authorizations and re-authorizations
• Communicate with payers, case managers, and utilization review teams
• Ensure services are approved, compliant, and aligned with care plans
Case Management & Care Coordination
• Collaborate with hospitals, physicians, families, and internal teams
• Monitor patient cases for changes in condition or care needs
• Support continuity of care and smooth transitions across settings
Clinical Operations & Leadership Support
• Partner with and support the Director of Patient Care in daily clinical operations
• Provide clinical oversight and guidance to nursing staff
• Ensure compliance with state regulations, payer requirements, and agency policies
• Support quality assurance, documentation standards, and clinical workflows
