A Brief Overview

This role serves as a clinical liaison between the UH acute rehab facility, UH acute care hospitals and out-of-System acute care hospitals. The Clinical Liaison facilitates the identification and assessment of appropriate patients for admission to the acute rehab setting in order to provide the best recovery setting for the patient and to promote growth for the Acute Rehab unit. This position constantly interacts with Providers, Nurses, Care Transitions teams, Therapists, and patients and families in a warm, positive, friendly and professional manner.

What You Will Do
  • This role serves as a clinical liaison between the acute care hospitals and Acute Rehabilitation. The Clinical Liaison facilitates the identification and assessment of appropriate patients for admission to the acute rehab setting in order to provide the best recovery setting for the patient and to promote growth for the Acute Rehab unit. This position constantly interacts with Providers, Nurses, Care Transitions teams, Therapists, and patients and families in a warm, positive, friendly and professional manner.
  • Communicates daily with clinical staff and patients at various UH acute care hospital units
  • Communicates face-to-face with all potential patients and families and clearly articulates the benefits of the acute rehab setting
  • Facilitates family tours of acute rehab facility to help with transfer decision prior to admission
  • Reviews patients medical records including therapy assessments, diagnoses and test results to determine appropriateness of admission to acute rehab
  • Identifies patient insurance information for rehab Admissions Coordinator approval and verification purposes
  • Works closely with Care Transitions teams and Acute Rehab Admissions Coordinator to recommend and facilitate transfer to acute rehab in a timely manner, pending insurance (or other) approval of admission
  • Functions as a resource and educates referring providers, therapists and nurses on admission criteria, regulations, and benefits of acute rehab for certain patient diagnoses
  • Initiates and maintains positive relationships with Providers, nurses, therapists and patients.
  • Works with UH Marketing Department to promote public awareness of the acute rehab unit and helps create updated brochures, flyers and/or videos as needed.
Additional Responsibilities
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Qualifications

Education
  • Associate's Degree Graduate of an accredited school of nursing, physical, occupational therapy assistant programs (Required) and
  • Bachelor's Degree (Preferred)
Work Experience
  • 3+ years clinical nursing or therapy experience (acute care) or acute rehab setting (Required) and
  • 1+ years of experience in case management/discharge planning in an acute care setting (Preferred) and
  • knowledge of Medicare and other payer regulations (Preferred)

Licenses and Certifications
  • Licensed Practical Nurse (LPN), Ohio and/or Multi State Compact License (Required Upon Hire) or
  • Physical Therapist Assistant (PTA) in the State of Ohio (Required Upon Hire) or
  • Occupational Therapist Assistant (OTA) In the State of Ohio (Required Upon Hire)
  • Driver's License (Valid) (Required Upon Hire)
