We are seeking a Clinical Liaison to support continued growth across the home infusion space. This is an opportunity to combine your clinical background with a consultative, referral-driven approach while working closely with providers, discharge planners, and care teams to improve patient access and outcomes.
About the Opportunity
In this role, you’ll serve as a key connection point between acute care settings and home-based infusion services. You’ll collaborate with physicians, hospital staff, payers, and internal clinical teams to help coordinate smooth patient transitions and support continuity of care.
This position is ideal for someone who enjoys building professional relationships, educating referral sources, and making a direct impact on patient experiences.
What You’ll Do
Develop and maintain strong relationships with physicians, discharge planners, case managers, and healthcare providers
Identify appropriate patients for home infusion services
Coordinate transitions of care from hospital to home
Collaborate cross-functionally with clinical, pharmacy, reimbursement, and operations teams
Help develop individualized care plans and support referral growth initiatives
Educate referral partners on infusion services and patient care solutions
Serve as a trusted resource throughout the patient onboarding process
What You Bring
Current and unrestricted RN license
2+ years of experience within infusion healthcare or related clinical settings
Strong communication and relationship-building skills
Ability to work collaboratively in a fast-paced healthcare environment
Sales or business development experience is a plus
Passion for patient-centered care and improving outcomes
