Job Description

We are looking for an organized, personable, and adaptable Clinical Liaison to join our team. This position combines office operations, client coordination, and community relations. The role includes assisting with daily office needs, onboarding new clients, supporting scheduling and client communication, and building relationships with healthcare providers and community referral partners. The ideal candidate enjoys working with people, can balance competing priorities, and is comfortable transitioning between office responsibilities and community outreach. This is an opportunity for someone with a healthcare background and strong relationship-building skills to contribute to both the daily operations.

Primary Responsibilities

Office Operations and Client Coordination

  • Assist with daily office operations, including answering phones and responding to client, family, and caregiver inquiries.
  • Coordinate new client onboarding, including required documentation, communication, and preparation for the start of services.
  • Assist with client visits, follow-ups, scheduling needs, and other operational responsibilities as needed.
  • Communicate client updates and concerns to the appropriate team members.
  • Provide general office support and assist with changing priorities as operational needs arise.

Marketing and Community Relations

  • Build and maintain relationships with hospice providers, home health agencies, hospitals, skilled nursing facilities, discharge planners, and other referral partners.
  • Conduct community outreach visits to introduce our services, strengthen existing relationships, and develop new referral opportunities.
  • Follow up with referral sources and maintain consistent communication to support ongoing partnerships.
  • Represent the company at networking events, community meetings, and other outreach opportunities.
  • Track outreach activities and collaborate with leadership on referral development and business growth goals.

Required Qualifications

  • High school diploma or GED.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to manage multiple priorities and adapt to changing needs.
  • Professional demeanor and ability to build relationships with clients, families, and healthcare professionals.
  • Ability to work independently and collaboratively with the office team.
  • Valid driver's license, reliable transportation, and ability to travel for community outreach.

Preferred Qualifications

Healthcare experience is preferred but not required. Ideal candidates may include:

  • LPNs with hospice experience (strongly preferred).
  • LPNs with home health experience.
  • PTAs or OTAs with home health experience.
  • PTAs or OTAs with skilled nursing facility (SNF) experience.

Candidates with other relevant healthcare, care coordination, or relationship-building experience will also be considered.

We are an equal opportunity employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status, or any other characteristic protected by law.
