Clinical Liaison — San Francisco Bay Area

If you’re a nurse who thrives in a fast-paced, relationship-driven environment and enjoys being the bridge between hospital teams and home care, this is an opportunity to play a key role in improving patient outcomes—especially for those requiring Parenteral Nutrition.

This role sits at the intersection of clinical expertise, coordination, and growth, partnering closely with providers, patients, and internal teams to ensure seamless transitions from hospital to home infusion.

What You’ll Do

  • Serve as the primary point of contact for patients transitioning from hospital to home infusion

  • Coordinate Parenteral Nutrition therapy initiation, including patient assessments and care planning

  • Build and maintain relationships with physicians, hospitals, and key referral sources

  • Partner cross-functionally with sales and operations to drive referrals and streamline onboarding

  • Educate patients and caregivers on therapy, equipment, and self-care best practices

  • Provide clinical insight and support to providers, including therapy recommendations and patient-specific considerations

  • Support bedside teaching and collaborate across territories when needed

  • Track activity and opportunities through CRM tools

  • Assist with local meetings, events, and discharge coordination efforts

  • Ensure accurate documentation aligned with regulatory standards

  • Manage your territory with a focus on growth, execution, and patient impact

What You Bring

  • Active RN license (California or compact)

  • Willingness to obtain additional licensure in WA, OR, and ID

  • Strong clinical background with the ability to communicate effectively across stakeholders

  • Experience or interest in home infusion, nutrition support, or care transitions

  • Relationship-driven mindset with the ability to support both clinical and growth initiatives

  • Comfortable working autonomously while collaborating across teams

  • Willingness to travel within territory (and occasionally beyond)

  • Valid driver’s license and proof of insurance
