The primary purpose of the Clinical Liaison is to serve as a clinical leader and centralized point of contact responsible for assessing patient eligibility, supporting goals-of-care discussions, and coordinating the timely and appropriate transition of patients from the Palliative Care Division to Hospice Care. This role ensures continuity of care, regulatory compliance, and alignment with patient and family goals as clinical needs evolve.
The Nurse Navigator communicates, coordinates, and directs patient care transitions in collaboration with palliative care providers, hospice teams, attending physicians, facility partners, hospitals, and internal referral and admission teams. The role supports high-level accounts and referral relationships by facilitating referral management, community education, and outreach initiatives that promote awareness of hospice eligibility and end-of-life care options.
In this role, the Nurse Navigator assists with and leads goals-of-care conversations; provides education to patients, families, and healthcare partners regarding disease progression, hospice eligibility criteria, and end-of-life services; and fosters effective communication between the palliative care team, hospice interdisciplinary team, and facility staff. The NN ensures appropriate follow-up with facility staff, physicians, hospitals, patients, and families to support informed decision-making and seamless care transitions.
The NN conducts visits to patient homes, facilities, hospitals, and physician offices as needed; collaborates closely with referral and admission coordinators; and coordinates the provision of end-of-life resources and services on behalf of patients and families, ensuring compassionate, patient-centered, and clinically appropriate care throughout the transition process.

Duties and Responsibilities
- Successfully attains weekly referral and admission goals per sales strategy.
- Participates in the weekly interdisciplinary team meeting.
- Perform administrative duties such as signing consents, completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
- Creates and maintains a patient/referral contact list and build relationships with key customers
- Perform chart audits to identify patients that meet hospice criteria.
- Maintains the hospice watch list and high acuity list updating as needed.
- Serve as the Palliative Care RN SME providing ongoing education and acting as a resource for clinical and non-clinical staff.
- Review all incoming referrals to assess acuity and follow-up needed.
- Collaborate with Providers to ensure patients are at the most appropriate level of care.
- Document all patient activities in the EMR and communicate updates to the clinical Team.
- Provide direct education to patients and families about Providence Care continuum of services.
- Engage in Goals of Care conversations and palliative care, care planning.
Personnel Functions
- Makes routine patient, facility, doctor, and hospital visits to directly market Providence’s services. Ensures that patients are in the correct level of care.
Working Conditions
- Works in office area(s) as well as the community, patient homes and long-term care facilities.
- Moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with physicians, physician's staff, patients, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
- Is subject to hostile and emotionally upset patients, family members, personnel, and visitors.
- Communicates with the medical staff, nursing personnel, and other department supervisors.
- Works beyond normal working hours and on weekends and holidays when necessary for special projects.
- Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
- Is involved in community/civic health matters/projects as appropriate.
- Attends and participates in continuing educational programs.
- Is subject to injury from falls, etc., throughout the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
QualificationsEducational Requirements
Must possess, as a minimum, a Nursing Degree from an accredited college or university and be licensed as a Registered Nurse in the State of South Carolina.
Experience
Must have, as a minimum, two (2) years of experience as a nurse in a hospital, long-term care facility, or other related health care facility. Clinical and Sales experience in a hospice, healthcare, or similar setting preferred.
Specific Requirements
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, patients, family members, visitors, physicians, government agencies/personnel, and the general public.
- Must be knowledgeable of hospice nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to hospice
- Must be knowledgeable of hospice practices and procedures, as well as laws, regulations, and guidelines that pertain to hospice care.
- Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
- Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
- Must not pose a direct threat to the health or safety of other individuals in the workplace.
- Must be comfortable, confident and experienced with Goals of Care conversations.
