🌟 We’re Hiring: Clinical Lead

Permanent, Full Time + on call telephone support on a rota system

Field based with frequent travel across your allocated areas

ÂŁ49,000-ÂŁ53,000 + ÂŁ3,000 car allowance

Are you passionate about delivering exceptional care and making a real difference in the community? We’re looking for Clinical Leads to join our Learning Disability, Autism and Mental Health Service to manage a caseload of community care packages.

đź’ˇ About the Role

In this role, you will:

  • Undertake initial holistic nursing assessments in partnership with the service user and their family.

  • Develop individualised care plans in partnership with the service user, their families and other health care professionals to ensure goals are patient focused, realistic and achievable.

  • Produce standard and bespoke risk management plans for service users, ensuring any identified high risk is escalated and acted upon, working with other stakeholders to plan, monitor and review effectiveness of any plan in place.

  • Be responsible for the delivery of client specific clinical training for staff within packages.

  • Be accountable for ensuring all clients allocated are regularly reviewed in accordance with company policies, ensuring high standards of clinical practice and reflecting current organisational and national guidelines and standards.

This is a role where your nursing skills and knowledge will really make a difference.

📚 Experience and Qualifications We’re Looking For

  • A Learning Disability or Mental Health Nursing degree with an active NMC pin

  • Experience with working with people who challenge services

  • Proven experience in leading teams to support individuals

  • A drivers license with access to a vehicle
