As a Clinical Initiatives Manager you’ll contribute to our success in the following ways:

  • Participates in strategic programs development related to innovation in clinical initiatives.
  • Oversight for Elara’s clinical initiatives implementation – oversees the functionality of the implementation process, provides insight, training and audits of patients’ records.
  • Provides clinical leadership and functions in the role of expert clinician, educator, and consultant.
  • Manages the needs of the patients with complex needs and delivers patient care at an expert level.
  • Provides education to the patient/family and members of the health care team
  • Monitors quality outcomes and enhances care practice through evidence-based policies and procedures, research, and implementation of new techniques, equipment, products, and processes for safe, cost-effective, quality care, and provision of care for the patients with multiple clinical needs.
  • Assists with effective care coordination within Elara and with outside entities to optimize outcomes.
  • Collaborates with the Education, Clinical Programs, and Quality team on projects focused on lowering hospitalizations, improving patient care and clinical outcomes. .

What is Required?

  • Bachelor’s Degree in Nursing.
  • Minimum five (5) year’s clinical experience in SHH.
  • Previous experience with kidney disease patients preferred.
  • Current Licensed Registered Nurse (RN).
  • Must be willing to obtain a compact RN license or multiple state licenses if does not reside in the state that allows compact licensure
  • Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
  • TRAVEL: ground and air travel – 50% travel. Frequency may vary depending on the needs of the organization.
