POSITION: CLINICAL FACILITIES MANAGER

PURPOSE OF POSITION

To serve as a member of the team to provide appropriate and adequate professional nursing care as prescribed by the physician in compliance with applicable RN licensure and agency policies and procedures to those patients who desire to receive hospice care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Listed below is an outline of the essential duties and responsibilities that will be required. Administrative authority, responsibility, and accountability necessary to perform the assigned duties under the supervision of the Chief Clinical Officer are delegated to the Clinical Facilities Manager.

DUTIES AND RESPONSIBILITIES

Collaborates with the Chief Clinical Officer, Admissions Department, and Executive Leadership

POSITION SUMMARY

The Clinical Facilities Nurse Manager is a leadership role responsible for driving clinical excellence, strengthening facility partnerships, and supporting hospice census growth within assigned nursing facilities.

This position requires full accountability for:

  • Management of a patient caseload
  • Clinical leadership and oversight of hospice staff in facilities
  • Active development of facility relationships to generate appropriate hospice referrals and admissions.

The RN is expected to operate with a high level of autonomy, initiative, and accountability, ensuring both quality patient outcomes and measurable contribution to organizational growth.

ESSENTIAL RESPONSIBILITIES

  1. Clinical Caseload Management (Non-Negotiable)
  • Maintains a consistent and appropriate hospice patient caseload.
  • Completes timely admissions, recertifications, routine visits in accordance with agency standards.
  • Ensures all documentation is accurate, timely, and compliant with regulatory requirements.
  • Demonstrates strong clinical judgment in symptom management, decline identification, and care planning.
  • Actively participates in IDT and ensures follow-through on all care plan interventions.
  1. Facility Leadership & Staff Accountability
  • Serves as the clinical lead for all hospice care delivered within assigned facilities.
  • Holds hospice staff accountable for:
  • Visit frequency compliance
  • Documentation standards
  • Quality of care delivery
  • Provides real-time coaching, redirection, and escalation when performance concerns arise.
  • Identifies and addresses gaps in care, communication, and coordination within facilities.
  • Ensures hospice presence in facilities is professional, responsive, and highly visible.
  1. Facility Relationship Ownership & Growth Expectations
  • Takes full ownership of assigned facilities as key referral sources.
  • Establishes and maintains strong, consistent relationships with:
  • DONs
  • Administrators
  • Social workers
  • MDS coordinators
  • Floor nurses
  • Actively identifies hospice-appropriate patients through:
  • Chart reviews
  • Facility rounding
  • Participation in care conferences
  • Provides ongoing education to facility staff on hospice eligibility and services.
  • Drives referral activity by ensuring hospice is top-of-mind and trusted within each facility.
  • Expected to contribute to sustained census growth and increased referral conversion rates.

QUALIFICATIONS

  • Current Registered Nurse (RN) license in Texas or Compact State
  • Minimum 2–3 years hospice experience required.
  • Experience in nursing facilities strongly preferred.
  • Demonstrated leadership and accountability in prior roles.

POSITION EXPECTATION STATEMENT

General

a. Shall know and conform to the Texas Nursing Practice Act as delineated in Title 3, Subtitle E, Chapter 301 of the Texas Occupations Code, the Board of Nurse Examiner’s standards, rules and regulations as delineated in Title 22, Part 11 of the Texas Administrative Code, as well as all federal, state, or local laws, rules or regulations affecting the nurse’s current area of nursing practice.

b. Maintains Licensure as required by Title 3, Subtitle E, Chapter 301 of the Texas Occupations Code and Title 22, Part 11 of the Texas Administrative Code.

c. Notifies Hospice immediately of any pending action against the license or pending suspension or any suspension or action taken against the license.

d. Has knowledge of and follows Hospice policies and procedures.

e. Maintains a professional appearance.

f. Displays a consistent positive attitude.

h. Adheres to Hospice of El Paso Mission and Value Statement.

i. Performs other duties as assigned.

  1. Subject to hostile and emotionally upset patients, family members, etc.
  2. A willingness to work beyond normal working hours, and in other positions temporarily, when necessary.
  3. Subject to falls, burns from equipment, infectious disease, odors, etc., throughout the workday.
  4. Exposure Level – Category I. Direct contact with blood, other bodily fluids, and potentially infectious materials to which standard precautions apply. Personal protective equipment is made available and must be worn.

  • Current Registered Nurse (RN) license in Texas or Compact State
  • Minimum 2–3 years hospice experience required.
  • Experience in nursing facilities is strongly preferred.
  • Demonstrated leadership and accountability in prior roles.

PHYSICAL REQUIREMENTS

  1. Sits, bends, lifts, and walks intermittently during working hours. Can sit for prolonged periods.
  2. Can reposition up to 50 pounds of weight.
  3. Possesses sight/hearing senses or uses prosthetics that enable these senses to function adequately, so that the requirements of this position can be fully met.
