Overview:

The Clinical Director of Home Health provides oversight and coordination of the clinical and operational divisions of the home health agency. This role manages all patient care services and personnel including nursing, home health aides, medical social work, physical therapy, occupational therapy, and speech-language pathology staff and contractors. The Clinical Director ensures that patient care is coordinated effectively through appropriate assignments, referrals, and the development and implementation of individualized plans of care.

The Clinical Director assists with oversight of safety and infection control programs, chart audits, and medical supplies while ensuring that medical records are maintained appropriately. This role assures that all clinical disciplines function according to licensure requirements, state law, and Medicare/Medicaid standards for Home Health services. The Clinical Director works closely with the Administrator to support the overall operation of the agency and may act on behalf of the Administrator when authorized.

Home Health care focuses on delivering skilled nursing and therapy services to patients in their place of residence. The goal of home health services is to promote independence, recovery, and improved health outcomes while allowing patients to remain safely in their homes.

Shift: Full Time 8a-5p

Responsibilities:

• Provide clinical leadership, supervision, and support to interdisciplinary home health teams, including nursing, therapy, social work, and aide services.
• Oversee patient care delivery, ensuring individualized care plans are developed, implemented, and updated to meet patient needs and regulatory requirements.
• Ensure compliance with Medicare, Medicaid, state licensure regulations, agency policies, and home health standards of practice.
• Collaborate with physicians, clinicians, referral sources, and community partners to coordinate high-quality patient care and successful outcomes.
• Lead Quality Assessment and Performance Improvement (QAPI) initiatives, infection control programs, and outcomes-based quality improvement efforts.
• Recruit, develop, coach, and evaluate clinical staff while promoting ongoing education, professional development, and employee engagement.Manage staffing, scheduling, productivity, and caseload distribution to support quality care delivery and operational efficiency.
• Oversee clinical documentation, medical record compliance, and the maintenance of policies and procedures to ensure regulatory readiness.Assist with budget planning, resource utilization, census management, and performance monitoring to achieve financial and operational goals.
• Serve as a key operational leader and act on behalf of the Administrator when delegated, supporting agency growth, patient satisfaction, and organizational success.
• Provide strategic and clinical leadership for home health operations, ensuring exceptional patient care, workforce effectiveness, regulatory compliance, quality outcomes, and organizational success.

Qualifications:

Education: Graduate of an accredited nursing program or licensed physical therapy program. Bachelor’s degree in nursing, physical therapy or related healthcare field preferred.

Licensure/Certification: Licensed Registered Nurse or Physical Therapist in the State of Ohio.

Valid Ohio driver’s license and reliable transportation that is insured according to state requirements.

Experience: Minimum three (3) years of clinical experience, with at least one (1) year in home health required. Minimum two (2) years of supervisory or management experience.
