Job Description

FutureCare Clinical Dietitians are valued Interdisciplinary Team members who provide residents with personalized nutrition care through timely nutrition assessments and individualized nutrition interventions. FutureCare Pineview delivers a full continuum of services, including VitalStrong Rehabilitation, long-term care, and dialysis. We also offer a progressive pulmonary care unit specializing in ventilator care and weaning.

Salary Range $85,000-$97,000

Salary Disclosure Statement

The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately. Offers will consider such factors as overall experience, job-related qualifications, location, certifications/training, etc.

Responsibilities

  • Work closely with the Interdisciplinary Team to ensure residents receive the best clinical care possible.
  • Provide nutrition services and medical nutrition therapy integrated with the patient's medical needs.
  • Assess and identify the nutritional status of residents using validated screening tools.
  • Complete timely nutrition assessments and Minimum Data Set (MDS) assessments as required.
  • Plan, develop, implement and monitor the nutrition plan of care for each resident.
  • Provide nutrition recommendations for patients reliant on enteral nutrition support.
  • Provide nutrition education to patients and families.
  • Utilize current standards and evidence-based practices.
  • Prioritize patient-based care and residents’ rights.
  • Complete monthly quality assurance and process improvement audits.

Qualifications

  • Current Registered Dietitian license in the state of Maryland.
  • Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field.
  • At least 2 years of experience as a Dietitian, preferred.
  • Previous experience working in a skilled nursing facility is highly desired.
  • Knowledge of the Nutrition Care Process and ability to apply all steps of the process.
  • A strong desire to deliver care in an empathetic, compassionate way.
  • Superb organizational and time management skills.
  • Decision making, problem solving, and collaboration skills are a plus.
  • Excellent interpersonal, written, and verbal communication skills.
  • Strong leadership skills and ability to work collaboratively with team members.
