The Clinical Diabetes Educator - Nurse provides state of the art diabetes self-management education (DSME) in a variety of settings, to persons with diabetes (PWD) and their families/care-partners, helping to optimize their diabetes management.

Clinical Diabetes Educator – Nurse

The Clinical Diabetes Educator – Nurse provides diabetes self-management education (DSME) to persons with diabetes (PWD) and their families/care partners to support effective diabetes management and overall health.

This position is primarily based in the inpatient setting, providing education to hospitalized patients and collaborating closely with the BIDMC inpatient care team. Occasional responsibilities may extend to outpatient or other clinical settings as needed.

Key Responsibilities

• Provide individualized diabetes education, assessment, care planning, and ongoing support to patients and families.
• Educate patients on diabetes management, nutrition, exercise, glucose monitoring, medications, and prevention of diabetes-related complications.
• Provide training and support on diabetes technologies, including insulin pumps and continuous glucose monitors.
• Monitor patient progress and adjust nursing care and education plans as appropriate in collaboration with the healthcare team.
• Provide daily inpatient diabetes education at BIDMC, including individualized assessments, hands-on device teaching, medication management, and safety training.
• Administer medications safely and accurately and provide patient education related to medication use.
• Provide phone triage and participate in emergency response on a rotating on-call basis.
• Lead individual and group education sessions and connect patients with appropriate community and support resources.
• Collaborate with physicians, nurses, dietitians, educators, and other healthcare professionals to ensure coordinated patient care.
• Develop and update diabetes education curricula, programs, presentations, handouts, and other educational materials in accordance with applicable standards and best practices.
• Participate in clinical diabetes education research and quality improvement efforts.
• Mentor less experienced staff, students, and other healthcare professionals.
• Maintain required professional registration and certification and remain current on diabetes research, treatments, and technology through continuing education.
• Complete required documentation, medical records, and billing in accordance with Joslin policies and applicable education standards.
• Participate in professional organizations, outreach, lectures, workshops, and conferences as appropriate.
• Perform other duties as assigned.

Required Job Qualifications:

Minimum education required

• Bachelor’s degree in Nursing

Minimum experience required

• Minimum of three years’ experience as a nurse, preferably at least one year in community health or ambulatory care setting, or diabetes care and education/counseling of patients with chronic disease

Licensure and/or Certification required:

• Registered Nurse, Massachusetts License
• BLS Certification
• Certified Diabetes Care and Education Specialist (CDCES) certification

Knowledge, skills and abilities required:

• Proficient in Microsoft Office Suite applications
• Excellent written and oral communication – including public speaking
• Must be detail oriented, and self-directed, with the ability to plan, organize, and execute multiple responsibilities through completion.
• May require some travel.

Preferred qualifications

• Master’s Degree in Nursing
• Experience training in the use of diabetes technology
• Bilingual - ability to speak Spanish a plus

This multi-disciplinary approach includes but is not limited to physicians, nurse practitioners certified diabetes educators who are nurses, nutritionists and exercise physiologists as well as Joslin care coordinators, behavioral health specialists and administrative coordinators who are directly involved in the development of the plan of care for patients in collaboration with patients/families and caregivers. The care plan is the primary document of care for patients and each member of the team is responsible for contributing to and maintaining an accurate and patient centered plan that is shared with patients/families/caregivers and referring health care providers and other specialists. All members are essential in the provision of the Joslin Care model and agree to function in a manner that is respectful and thoughtful of one another, patients/families/care givers and other health care professionals and employees.

Pay Range:

$37.00 - $95.74

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
