Schedule:

  • Monday-Friday
  • 8 hour shifts
  • No holidays or weekends

Purpose:

  • Independently leads the planning and provision of professional nursing care for patients in the inpatient and outpatient settings in accordance with the medical and nursing plans of care and established policies and procedures. Provides patient/family centered care, acting as partner
  • and adviser, assists and supports patient and families to optimally manage their health care, respecting their culture and values, individual needs, health goals and treatment preferences.
  • Concurrently and retrospectively ensures the patient's progress in the acute and skilled episode of care through post discharge and is quality driven with being efficient, cost effective, with a safe discharge plan.
  • Works collaboratively with all members of the multi-disciplinary team to ensure
  • patient needs are met and care delivery is coordinated across the continuum, as well as appropriately reimbursed by payers, and that resources are efficiently and effectively utilized.
  • Provides support to staff, physicians, families, patients and community members relating to child and dependent adult abuse, end of life care, and other community and facility needs.
  • Applies critical reasoning and astute clinical judgment in order to expedite appropriate care and treatment, especially given that the patient may be present with complex problems or potentially life threatening conditions.
  • Uses critical thinking in clinical problem-solving relative to the scope of practice, knowledge, competency and experience. Facilitates continuity of care
  • using the nursing process, multidisciplinary collaboration, and coordination of appropriate health care services and community resources across the care continuum.
  • Interacts with patients during face-to-face encounters or through a variety of telecommunication strategies, often establishing long-term relationships. Focuses on patient safety and the quality of nursing
  • care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, performing procedures, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient to navigate the health care system, and evaluating patient outcomes.
  • Designs, administers, and evaluates nursing services
  • within the organization in accord with relevant Federal requirements, state laws and nurse practice acts, regulatory standards, and institutional policies and procedures.
  • Participates in regular team meetings and continuous quality improvement activities.
  • Maintains knowledge of care teams for coordination of patient care, self-management, population management and communication skill.

This role will also lead the Infusion unit and oversee and administer various types of infusion therapy to oncology and non-oncology patients' blood components, fluid and electrolyte replacements and other oncology treatments as prescribed. The coordination of preauthorization, scheduling and management of infusion orders. Maintains a knowledge of reimbursement procedures. Completes all chemotherapy charge forms. Performs nursing assessments and triage of patient care needs for new and ongoing patients. Assesses patient

physical condition as well as their response to treatment, communicates with physician of any changes and ensures all patient needs are met. Educates patients on their condition, treatment, medications being administered, etc. Records patient information including vital signs.

General Requirements:

  • Bachelor's Degree in Nursing
  • Registered Nurse in Iowa
  • More than 1 year of experience
  • Basic life support, ACLS and PALS
  • Preferred: Master's degree; Certification in Case Management, Infusion non clinical and clinical, Professional Utilization Review or Managed Care; DRG/CPT knowledge; Interqual Admission/Continued Stay criteria knowledge including skilled care
