The Clinical Pharmacist Coordinator will perform educational and clinical services for the facility. They will be a drug information resource for the nurses, physicians, patients, and families. In addition, they will use critical thinking skills and perform independent decision-making as needed for the furnishing of safe and rational medication therapy. Assist upper-level management with optimization of patient care in a safe and cost-effective manner.
Primary Duties
· Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
· Attendance on medical rounds (for decentralized shifts) with documentation of pharmacist interventions based on availability.
· Assist in the Develop and implement facility-specific protocols, guidelines, and reference documents related to medication therapy management that minimize the potential for medication errors
· Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.
· Provide support to Staff Pharmacist and able to provide all general responsibilities of a Staff Pharmacist, as needed.
· Demonstrates independent leadership and professionalism with respect to the oversight of pharmacy technicians, pharmacy students, and pharmacy interns.
· Ensure pharmaceutical care programs and medication therapy management are appropriately integrated throughout the institution.
· Assist department leaders in ensuring and maintaining compliance with regulatory standards (TJC,FDA,CDPH,CMS,CABoP,ISMP)
· Contributes to department research for both pharmacy students and residents.
· Participate in the education and training of healthcare professionals (e.g. physicians, nurses, pharmacy technicians, and students) in relation to medication use.
· Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.
· Supports the practice of pharmacy in accordance with all applicable regulations and upholds pharmacy policy/procedures as appropriate.
· Deliver medication therapy management and provide support to ensure pharmaceutical care programs are appropriately integrated throughout the institution.
· Establishes and maintains professional relationships with colleagues, physicians and other care providers.
· Supports department operations and assures responsibility for professional development.
· Assist s Pharmacy Management by ensuring the most effective and cost-efficient medications are on formulary and implementation of therapeutic substitutions where appropriate per Tenet Policy.
Position Qualifications
Physical demands:
· Moderate physical effort (lift/carry up to 25 lbs)
· Manual dexterity and mobility
· Occasional reaching, stooping, bending, kneeling and crouching
· Prolonged standing/walking
· Occasionally lifts supplies/equipment
Working Conditions:
· Handles emergency or crisis situations
Required:
· Possess a valid CA pharmacist’s license (maintain current at all times)
· PharmD or BS in Pharmacy
· Board Certification in Pharmacotherapy (BCPS)
· Pharmacy practice residency or equivalent hospital experience (3 postgraduate years).
· Good written and verbal communication skills.
· Demonstrate good command of basic mathematical calculations.
Preferred:
If routinely staffed in critical care areas one or all the following advanced life support certifications will be required within 90 days of hire, as applicable
· American Heart Association ACLS
· PALS
· Excellent written & verbal communication skills
· Completion of an ASHP residency program
· Certification through the board of pharmaceutical specialties or equivalent
· Collaborative practice experience using protocol management skills
· Automatic Dispensing Machine experience (Omnicell, Pyxis, etc.)
· Familiarity with CPOE, Cerner, or other electronic documentation systems
· Office (Word, Excel, PPT) experience
