Clinical Coordinator- Neurosurgery Clinic-(26004533)

UI Health Care Department of Nursing is seeking a 100% Clinical Coordinator to work in the Neurosurgery Clinic.

Position Responsibilities:

  • Collaborateswithnursing,medicalstaffandotherhealthprofessionalsinthedevelopmentandreviewofstandardsandinplanning,implementingandevaluatingpatientcareforthe adult urology patientpopulationwithregardtotheconditionandageofthepatient.Providesdirectnursingcarewhennecessary.Followsuniversalprecautions.

  • IndependentlycoordinatesandfacilitatespatientcarebetweenmultipledisciplineswithinboththeconfinesofUI Health Careandthecommunity(includinghomehealthagencies,referringphysiciansandothercommunitysupportservices).Facilitatescontinuityofcare.

  • Coordinatespatientcaretobetterutilizesupportiveservicesthroughpatientvisit,pre-schedulestesting,followsupwithtestresultstobothpatientandphysician,obtainsmedicalhistory,coordinatesfollowupappointments,providesmedicationanddiseaseprocess,teaching,etc.Participateintheongoingmanagementofthepatienttofacilitatecareofpatientinatimelymanner.

  • Facilitatescareofadult urologypatientsrequiringadmission,coordinatingnecessaryadmissionprocedures,preandpostprocedurecare.

  • Actsasaclinicalresourceorconsultantforadult urologypatientpopulations withregardtotheconditionandageofthepatients.

  • Participatesinimplementationandrequirementsofmanagedcareprogramsforadult urologypatientpopulations.

  • Operatesautonomouslyincommunicatingwithmembersofthehealthcareteamdirectingpatientcareandprovidingcontinuityofcareacrosssettingsthroughoutthephasesofcare.

  • Autonomouslycommunicateswithreferringphysicianandhealthcarefacilities.

  • Revisespatientcareregimeviathetelephonewithinprotocolsdevelopedcollaboratively.PerformstelephonetriageofpatientcallstotheUrologyservice andfacilitatesmedicationrefillsasneeded.

  • Providesleadershipfordirectpatientcareprogramsforurologypatientscollaboratingwithhealthcareprofessionals.

  • Participatesindecision-makingprocessatunit/division/departmentlevelofissuesthatimpactclinicalpractice.

  • Participatesin/providesleadershipfordivisional,departmentaland/orintrainstitutionalandcommunitycommitteesandprojects.

  • Participatesintheongoingimplementationofcurrentregulatorystandardsinordertomeetstateandnationalagencyaccreditation.

  • Incorporatestheroleofpatientcareprovider,advocate(patient,family,andhealthcareprofessional),consultant,educator,researcherandqualitymanager.

  • Participatesinthedevelopmentandimplementationofdivision/unitqualityassessmentandimprovementprogramusingappropriateriskandqualitymanagementdata.

  • Usesrelevantnursingresearchtoimproveclinicalpracticeandinvestigatenursingcareproblemsrelatedtoaspecificpatientpopulation.

  • Participatesindevelopmentandconductofresearchactivitiesinitiatedthroughrecognizedneedinareaofspecialtyandasdirectedwithotherdisciplines.

  • Designandimplementpatient/familyhealtheducationactivitiesforurology patientsrecognizingindividualizedneed.

  • Providespatienteducationinthe Urologyclinicthatincludes:urologyprocedures,management of chronic urological disorders, perioperative and post-operative management, preventative urologic care.

  • Assesslearningneedsofpatientsandfamiliespost-opproceduresandprovides

  • appropriateeducationrelatedtodiagnosis,procedureperformed,riskfactormodification,andtreatmentregimens.

  • Facilitateslearningandprovidesconsultationtothoseinexternalagencies,communitiesandtootherhealthcareprofessionals.

  • Supportsimplementationandevaluationofstaffeducationandorientationprograms,coordinatingandpresentingthoserelatedtoUrology.

  • Facilitatestheeducationalexperienceforhealthcarestudentsandmonitorsthecaredelivered.

  • Provide direction, assignments, evaluative feedback, coaching and counseling to staff, students and residents to assure outcomes are achieved.

  • Performsclinicalduties,specialprojectsandothertasksasneeded.

    Percent of Time: 100% (40 hours per week)
    Schedule: Monday-Friday, 8-hour shifts within 7:30-5pm, may be variable start and end times to meet clinic demands. No nights, weekends, holidays, or call.

Qualifications

Education

  • A Baccalaureate Degree in Nursing is required

Required Qualifications
• Current license to practice Nursing in Iowa.
• Minimum 1 year related experience
• Demonstrated experience of coordinating care along the healthcare continuum.
• Demonstrated experience in patient education.
• Excellent written and verbal communication skills.
• Demonstrates positive verbal communication skills.
• Experience working with an interdisciplinary team.
• Proficient in computer skills (i.e. word, excel, powerpoint, access).
Desired Qualifications
• Neurosurgical patient experience preferred.
• Previous experience managing operations (coordinating/scheduling patient visits) highly desired.
• One year of current experience at University of Iowa Health Care highly desired.
