This role is to support the development, growth and day to day operations of the Nurses’ Health Program (NHP). The NHP is a voluntary and confidential support program for nurses across Ontario who may be experiencing mental health or substance use challenges. It focuses on early intervention and recovery, helping nurses access care while ensuring safe practice.

The program provides end-to-end support, from intake and assessment to individualized care plans, monitoring, and return-to-work support. Overall, the goal is to support nurses in their recovery journey while maintaining professional accountability and protecting patient safety.

The role requires carrying a case management file workload, leadership of quality assurance activities, administrative responsibilities, and collaboration with the case managers, physicians, program manager and the medical director as part of the NHP.

Key Responsibilities:

The Clinical Co-ordinator:

  • Case file management in the monitoring of participants enrolled in the program.

  • Collaboration with all stakeholders – case managers, physicians, program leaders and community providers.

  • Works closely with the program manager to ensure effective and efficient program operations.

  • Tracks and monitors key performance indicators.

  • Reviews case files and provides advise, coaching and feedback to the team.

  • Acts as a lead contact and liaison with internal and external stakeholders.

  • Summarizes complex case findings, facilitates case conferences and escalates issues appropriately

  • Acts as a clinical resource for the team that supports ongoing education, knowledge translation, and practice excellence.

  • Participates in the collection and analysis of business benchmarks and program evaluation metrics

  • Coaches, mentors, and develops other health professionals within the program.

  • Participates in continuous quality improvement and evaluation.

  • Contributes to the overall vision/mandate of the Program.

  • Supervises students as required. 

Key Qualifications:      

  • Regulated Health Professional registered with relevant regulatory body (e.g. Ontario College of Social Workers and Social Service Workers, College of Occupational Therapist of Ontario, College of Nurses of Ontario, etc)

  • Membership with relevant Association – preferred.

  • Minimum 5 years relevant experience working with patients with mental health conditions.

  • Excellent written and verbal communication skills.

  • Experience providing services with a strong quality improvement focus to third party payers.

  • Customer and client centered approach to delivering excellent service, clinically and administratively.

  • Evidence of continuing education, professional and personal development – required.

  • Reliable, flexible, creative, and self-directed professional; able to work effectively and collaboratively with an interdisciplinary team.

  • Exercises initiative and good judgment with ability to multi-task.

  • Strong ability to successfully coach, develop, and mentor allied health professionals and physicians to improve program quality.

  • Demonstrates excellent interpersonal, time management, and organizational skills.
