Title: Clinical Coordinator

Program: New Providence Women’s Shelter

Salary Range: $51,045-$55,045 annually

Program Description

The New Providence Women’s shelter is located within the Ramada Inn in West Midtown Manhattan and provides 100 emergency shelter beds for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, and housing placement assistance.

Essential Duties and Responsibilities

The essential duties of the Clinical Coordinator include but are not limited to the following activities:

  • Audit minimum of 8 charts per week (2 from each case manager) - provide detailed summary to the DSS following the audit.
  • Update Hospitalization, Chart Audit, and Incident Trackers.
  • Coordinates with outside medical providers, PRI medical team, DHS, and all other appropriate staff to support clients who are having surgical procedures. Updating all HIPAA forms for seamless communication.
  • Update Caseload breakdown and run demographics report’s weekly.
  • Supervise 2 Engagement Specialists with a minimum of Bi-Weekly supervision.
  • Attend and facilitate (as needed) PFP, staff meetings, and interdisciplinary meetings, as scheduled.
  • Assist clients with obtaining Entitlements & Vital documents.
  • Schedule medical and psych appts, including MedVan.
  • Review shift summaries as they are published.
  • Submits incidents in WeShare as needed and maintains incident reporting spreadsheet to ensure that follow ups are captured.
  • Works closely with the DSS to ensure that the Social Services program is running smoothly.
  • Write new intake or returnee case notes (only for those returning after a 30-day absence).
  • Attends Bi-Weekly supervision with Director of Social Services.
  • Provides support to case management staff ensuring ILPs, Incident Follow-ups, and charts are up to PRI and DHS standards.
  • Performs other duties as assigned by the Director of Social Services and Program Director.

Physical Activities:

While performing the duties of this position, the Clinical Coordinator (CC) must walk between all floors of the facility, climbing up and down staircases to meet with clients in the event the elevator is out of order. The CC must stand talking to clients and staff, sit for extended periods of time at the computer, and reach or squat to retrieve file records.

Qualifications

  • A bachelor’s degree in social work, public administration, psychology, education, sociology, occupational or physical therapy, or other related human services fields with a minimum of two (2) years of experience
  • An associate’s degree in the aforementioned fields with three (3) years of experience
  • A High School Diploma or GED equivalent with four (4) years of experience
  • Bilingual or multilingual a plus

  • Leadership: Demonstrated ability to manage and motivate staff to accomplish stated program goals and objectives while developing their individual and group skills.
  • Team Building: Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible.
  • Organizational: An ability to work independently and to adjust one’s schedule to accommodate the needs of the clients.
  • Communication: Excellent oral, writing, and listening skills.
  • Interpersonal: An ability to interface with clients as well as all levels of staff.
  • Computer: Knowledge of Case Management software (CARES). Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
  • Language: Bilingual or multilingual a plus.
