Short Description

The Clinical Coordinator will be responsible for patient enrollment services associated with the digital remote monitoring programs (COVID-19, Heart Failure, COPD, Diabetes). Roles and responsibilities could include digital platform enrollment and alert monitoring, telephonic patient outreach and education, coordination and escalation of care, and communication and collaboration with the providers and multidisciplinary team.

1. Facilitates patient identification, screening, and enrollment in RPM program 2. Collaborates with referring providers on patients enrollment workflow3. Communicates with providers, inpatient clinical directors, and other multidisciplinary team members status updates on RPM program enrollment as needed 4. Reviews patient agreement with patient and ensures signed, and scanned into patient EPIC electronic health record5. Orders digital devices for home delivery from platform partner and confirms receipt of device kit6. Coordinates with program manager to assist with securing phone service for identified patients7. Facilitates activation of patient’s MyChart account if not active8. Confirms device paring, patient education and understanding for utilization, and provides continued education where needed 9. Enrolls patient in platform partners electronic health record and updates as needed Monitors digitally transmitted vital sign results daily (M-F, excludes holidays)10. Supports patient with activation of patient’s MyChart account if not active11. Assists patient with remote patient monitoring adherence and program participation12. Outreaches to patients when vital signs are out of range or not transmitted telephonically or via video conference and follows escalation pathway

15. Documents all events and communications in patients CUHC electronic health record16. Collaborates with RPM Population Care Coordinator and clinical care team as needed17. Reinforces provider plan of care and patient centered care plan 18. Promotes chronic disease self-management19. Coordinates with RPM program manager and digital platform support services as needed for technical support20. Facilitates scanning of monthly vital sign summary into patient’s EPIC electronic health record

13. Documents vital signs weekly and when alerts out of range in the patient’s CUHC electronic health record14. Communicates with patient and or caregiver regularly and as needed

Experience Required

3-5 Years Preferred

Education Requirements

NJ LPN License and BLS American Heart Association
