The Clinical Case Manager, Private Pay will coordinate and oversee comprehensive care plans for clients receiving Nursing, PCHM, or HHA services. Collaborating with the Care Center Team, they will assess client needs, develop personalized care plans, and ensure that our Caregivers are delivering quality care. They will establish regular updates with Primary Nurses and determine when in-person visits are necessary.
The Clinical Case Manager may attend discharge planning and family meetings to facilitate home discharge and will be involved in intake to ensure appropriate care levels through discussions with staff and family. They will work with the Care Center to ensure HHAs meet skill requirements, address training needs with the Care Center and Education and Training, and help orient PC/HM and HCAs

Pay Range: The hiring range for this position is $70,000-75,000/annually. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
