- This position will support a region of retirement homes predominantly in the southwestern region of Ontario (Kitchener, Simcoe, Grimsby). On-site presence is required for a minimum of 4 days a week.
What You’ll Be Doing: As a valued member of the team, the Clinical Care Partner – Aspira Retirement demonstrates a commitment to the quality of resident care by providing services in accordance with the organization’s vision, mission, and values. Reporting to the Director, Clinical Services – Retirement, the Clinical Care Partner - Aspira Retirement plays a key role in strengthening clinical performance, supporting the implementation of care programs, and enabling communities to effectively and sustainably deliver higher levels of care.
RESPONSIBILITIES:
Clinical Operations & Community Support
- In collaboration with the Vice President, Regional Operations, General Managers, and Directors of Wellness, provide ongoing clinical support to communities to ensure care meets and exceeds organizational and regulatory expectations.
- Support the assessment of resident care needs and the development and implementation of individualized service plans.
- Monitor resident outcomes and incidents, supporting teams to implement appropriate interventions and prevent recurrence.
- Support communities in identifying and responding to changes in resident health status and care needs.
Care Program Implementation & Performance
- Support the implementation and ongoing execution of the Retirement care and wellness program across communities.
- Partner with community teams to ensure care delivery aligns with service plans, staffing models, and care fee structures.
- Promote and support care services to ensure alignment with operational goals and sustainable care revenue models.
- Identify opportunities to enhance care delivery, service offerings, and clinical practices as the portfolio evolves.
Quality Assurance & Risk Management
- Conduct regular site visits, chart audits, and quality reviews to assess compliance with organizational standards and regulatory requirements.
- Support communities in the development and execution of quality improvement and compliance action plans.
- Identify clinical risks and partner with community teams to implement mitigation strategies.
- Monitor performance indicators and support follow-up actions to improve outcomes.
Infection Prevention & Control (IPAC)
- Support the implementation of IPAC practices and protocols within communities.
- Conduct IPAC audits and provide guidance to ensure compliance with best practices and regulatory expectations.
- Support outbreak preparedness and response in collaboration with community teams and IPAC resources.
- Work in collaboration with the Manager, Infection Prevention and Control
Education, Coaching & Leadership Support
- Provide coaching, education, and mentorship to Directors of Wellness, Assistant Directors of Wellness, and community teams.
- Support orientation and ongoing development of clinical and wellness leaders.
- Facilitate education sessions related to clinical care, quality, IPAC, and regulatory requirements.
Regulatory Support & Compliance
- Support communities in meeting regulatory requirements across applicable jurisdictions (e.g., RHRA, Assisted Living regulations, Personal Care Home standards).
- Assist with preparation for inspections and support follow-up on findings.
- Promote a proactive approach to compliance and continuous improvement.
Collaboration & Stakeholder Engagement
- Develop strong relationships with internal and external stakeholders, including hospital partners and community healthcare providers.
- Collaborate with interdisciplinary teams to support clinical initiatives and quality improvement efforts.
- Participate in regional and organizational meetings and committees as required.
Operational & Financial Alignment
- Support communities in aligning care delivery with staffing models and operational expectations.
- Assist with monitoring care service utilization and identifying opportunities to improve efficiency and effectiveness.
- Partner with community leadership to balance resident care needs with operational and financial sustainability.
Who you are:
- Registered Nurse (RN), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN) in good standing with the applicable regulatory body
- Minimum 3–5 years’ experience in seniors’ care, retirement living, long-term care, or a related healthcare environment
- Leadership experience in a clinical or wellness role (e.g., Director of Wellness, Director of Care, Associate Director of Care)
- Strong knowledge of gerontology, dementia care, and resident-centered care practices
- Knowledge of applicable regulatory requirements across Retirement settings
- Experience with clinical assessments, care planning, and quality improvement processes

- Salary Range: $80,839 - $101,049 **Please note that individual salary is determined by factors such as job-related knowledge, skills and experience, as well as internal equity**
- Please note that this is a regional role in nature. The role required on-site presence at our Aspira Retirement homes for a minimum of 4 days a week. ** Travel reimbursement applies**
