We are seeking a Clinical Care Liaison to service as a key liaison in coordinating patient transitions, referral management, and community outreach. This position is integral to our business development efforts and requires strong relationship-building skills with patients, families, physicians, and healthcare providers. You will educate stakeholders on end-of-life care, facilitate hospice admissions, and ensure a seamless transition for patients and their families.

Key Responsibilities

  • Serve as the central point of contact for patient care transitions and business development.
  • Conduct goals-of-care conversations with patients and families.
  • Build and maintain relationships with referral sources, including hospitals, physicians, and long-term care facilities.
  • Educate healthcare professionals and the community on hospice and palliative care.
  • Conduct facility, home, and hospital visits to assess patient needs and facilitate referrals.
  • Achieve targeted weekly referral and hospice admission goals per sales strategy.
  • Participate in interdisciplinary team meetings and maintain accurate documentation.
  • Complete required administrative tasks, including consents, medical forms, reports, and patient records.



Qualifications

Education

  • Nursing Degree from an accredited college or university.
  • Active and unencumbered Registered Nurse (RN) license in the state of South Carolina.

Experience

  • Minimum 2 years of nursing experience in a hospital, long-term care facility, or related healthcare setting.
  • Clinical and Sales experience in a hospice, healthcare, or similar setting is preferred.

Skills & Abilities

  • Strong knowledge of hospice care practices, laws, and regulations.
  • Ability to independently manage tasks, make decisions, and solve problems.
  • Excellent interpersonal skills with patients, families, and healthcare professionals.
  • Strong organizational skills with the ability to document patient interactions accurately.
  • Excellent communication skills.
  • Flexibility to work in varied settings (office, facilities, patient homes) and adapt to urgent needs.
