At Prevea Health, the Clinical Care Liaison plays a vital role in closing care gaps and managing provider panels. This includes reviewing provider schedules, preparing for upcoming appointments, and ensuring patients receive necessary pre-appointment care. The liaison will conduct chart reviews to identify open care gaps and proactively reach out to patients via telephone or MyChart to coordinate any necessary steps for closing these gaps. By facilitating communication and supporting care coordination, the Clinical Care Liaison enhances patient outcomes and overall healthcare efficiency.

What you will do

  • Review provider schedules to conduct chart reviews before patient appointments.
  • Identify and address any gaps in care for patients under review.
  • Contact patients before their appointments to discuss care gaps and coordinate any necessary pre-appointment requirements.
  • Use dashboards to follow up and coordinate reassessments of specific care gaps, such as blood pressure checks.
  • Serve as liaison between providers and patients, ensuring effective communication through Electronic Medical Record (EMR) documentation.
  • Manage medication refill requests as needed, ensuring timely processing and coordination with providers.
  • Regularly review and update provider panels to maintain accurate and current patient information.
  • Facilitate the completion of required forms when needed, including workers’ compensation and insurance documentation, and any prior authorization forms on behalf of providers.


Education Qualifications

  • Graduate of an approved Medical Assistant program Required

Experience Qualifications

  • 1-3 years Current clinical experience as a Medical Assistant Required

Licenses and Certifications

  • One of the following Medical Assistant Certifications are required:
  • Certification as a Certified Clinical Medical Assistant (CCMA) through the National Healthcare Association Upon Hire Required
  • Certified as a Registered Medical Assistant from the American Medical Technologists (AMT) Upon Hire Required
  • Certified as a Medical Assistant from the American Association of Medical Assistants (AAMA) Upon Hire Required
  • CPR/Basic Life support for Healthcare Providers certification from approved program Preferred
