EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Referral and Assessment Coordinator to join the Jail Population Review Program.

Schedule: 9AM-5PM, Monday - Friday

What drives the core purpose of this job

The Clinical Assessor conducts timely, comprehensive clinical assessments of justice-involved individuals referred through the Jail Population Review initiative. The position evaluates behavioral health, substance use, psychosocial, and treatment needs; develops clinical formulations and recommendations; and prepares clear written reports to support consideration for Alternatives to Incarceration (ATI) and appropriate community-based services.

What are the key responsibilities

1. Conduct clinical interviews and comprehensive psychosocial assessments with incarcerated individuals referred for ATI and other legal consideration.
2. Assess mental health, substance use, treatment history, psychosocial functioning, strengths, barriers, and community-based service needs.
3. Review and integrate relevant collateral information, including medical, behavioral health, criminal legal, and treatment records.
4. Develop clinically sound formulations, treatment recommendations, and service plans responsive to each individual's needs and circumstances.
5. Prepare clear, objective, and timely clinical reports summarizing assessment findings, treatment needs, and recommendations.
6. Identify cases requiring additional consultation, psychological testing, risk assessment, or other complex clinical evaluation and consult with clinical leadership as appropriate.
7. Coordinate with program staff, attorneys, correctional personnel, treatment providers, and other stakeholders as needed to complete assessments.
8. Support warm handoffs to EAC Network ATI programs when individuals are accepted for services.
9. Participate in case conferences when needed.
10. Complete assessments, case notes, reports, and other required documentation within established program timeframes.
11. Maintain complete and accurate records and comply with confidentiality requirements, professional standards, agency policies, and program procedures.
12. Perform other relevant duties as assigned by program supervision and EAC Network administration.

What key knowledge, skills, and abilities are required to excel in this role:

1. Master's degree in Social Work, Mental Health Counseling, Psychology, or a related clinical field required.
2. Current New York State professional license or limited permit, such as LMSW, LCSW, LMHC, or equivalent, required.
3. Experience conducting behavioral health assessments, clinical interviews, psychosocial assessments, or treatment planning required.
4. Experience working with justice-involved or incarcerated populations strongly preferred.
5. Knowledge of ATI, forensic behavioral health, court-based services, mental health diversion, and co-occurring mental health and substance use disorders preferred.
6. Strong clinical judgment and the ability to synthesize interview findings, records, and collateral information into clear, objective written reports.
7. Excellent oral and written communication skills and the ability to collaborate effectively with criminal legal, behavioral health, and community service systems.
8. Ability to manage multiple assignments, meet firm deadlines, maintain professional boundaries, and work both independently and as part of a multidisciplinary team.
9. Proficiency with Microsoft Office, electronic records, databases, email, and other systems necessary to perform job responsibilities.
