FlexStaff is seeking a dedicated and detail-oriented Clinical RN Assessment Manager to join our client’s growing PACE team! This is a pivotal role where you will be at the forefront of ensuring our participants receive the precise care they need to live independently and with dignity.

As a Clinical RN Assessment Manager, you are the cornerstone of our participant's journey, playing a critical role in evaluating their unique needs and connecting them with life-changing services. You'll leverage your clinical acumen to conduct thorough assessments, drive accurate documentation, and collaborate with diverse teams to ensure seamless transitions and optimal outcomes.

Responsibilities:

  • Lead Comprehensive Assessments: Conduct crucial in-house and virtual assessments for prospective and enrolled PACE participants, expertly utilizing the Uniform Assessment System of New York (UAS-NY) to determine eligibility, care needs, and guide personalized service plans.
  • Be a PACE Ambassador: Articulate the unique benefits and services of our PACE program to participants, families, and potential referral sources, empowering them to make informed decisions about their healthcare journey.
  • Ensure Clinical Precision: Meticulously review and evaluate previous assessments, diagnoses, and medications, ensuring all documentation is accurate, up-to-date, and reflects any changes in condition within our electronic record system. Your keen eye for detail ensures continuity of care.
  • Drive Quality & Compliance: Champion Department of Health policies, UAS-NY regulations, and CMS/PACE guidelines, working closely with our quality review team to maintain the highest standards of documentation and care delivery.
  • Collaborate for Success: Partner seamlessly with our Intake, Enrollment, and Care Delivery teams, as well as site leadership and Interdisciplinary Teams (IDT), providing vital clinical insights that pave the way for smooth participant transitions and ongoing support.
  • Advocate for Participants: Develop proposed service plans, escalate critical changes to care teams, and act as a clinical resource post-enrollment, ensuring participants always receive appropriate and timely interventions.
  • Innovation & Improve: Engage in continuous quality improvement activities, identifying and advising on operational enhancements that boost efficiency, participant engagement, and overall program excellence.
  • Manage Your Impact: Maintain a consistent schedule of essential UAS-NY assessments (2-3 per day), expertly balancing your clinical responsibilities with timely documentation and follow-up.

Qualifications:

  • Active Registered Nurse (RN) license in New York State.
  • Proven experience in clinical assessment, particularly with the UAS-NY system (highly preferred).
  • Experience in long-term care, home care, or PACE programs is a significant plus.
  • Exceptional clinical judgment, critical thinking, and problem-solving skills.
  • Superior communication and interpersonal abilities, with a talent for educating and engaging diverse audiences.
  • Meticulous attention to detail and strong organizational skills.
  • Proficiency in electronic medical record (EMR) systems.
  • Ability to work independently, manage a dynamic schedule, and travel to various boroughs (some weekends required).
  • A true passion for serving the elderly population and improving their quality of life.
