The Clinic Social Worker (CSW) exercises independent judgment and professional discretion in the practice of case management to a selected group of patients. The CSW collaborates with the Mind Wellness Team and serves as advocate to help patients and families navigate care. The CSW is proactive and uses collaborative problem solving skills to meet the needs of the patients and Providers. The CSW provides social work case management services as needed and relies heavily on such resources as family, community, state and federal agencies as well as the patient’s resources to assist the patient’s and/or family’s return to a productive life.
Qualifications
The kind of position-related experience includes:
Social work experience is required and this experience in the medical field is preferred. The learning period recommended is six (6) months for proficiency in the majority of tasks. Candidate should possess excellent verbal and written communication skills, be able to build effective relationships with the healthcare team, and demonstrate critical thinking skills.
