The Client Services Site Coordinator (CSSC) will provide supervision of assigned Client Services staff and oversignet of the day-today operations of Client services programs at a designated PIHC Center. The CSSC will ensure that systems are compliant with grant requirements. Duties include but are not limited to ensuring the quality of support services to clients; ensure quality training of and for assigned staff, oversee case management and/or Rapid Entry linkage serves, and ensure that required standards and guidelines are in place.

Requirements

OF DUTIES

• Supervise assigned Lead Medical Case Managers (and Medical Case Managers as needed) to ensure day-to-day operations are met, adhering to the Ryan White Case Management Standards

• Supervise Enrollment Team, comprised of Rapid Entry Linkage Coordinator and Enrollment Case Manager, to ensure day-to-day operations are met

• Provide one-on-one supervision to assigned staff and facilitate monthly team meetings

• Monitor and coordinate provision of services via resource line, case management pool and other means

• Train new staff on the operations of enrollment and case management

• Assist team with completion of enrollments as needed

• Provide medical case management to patients as needed

• Monitor and evaluate care team tasks and goals

• Perform monthly audits to ensure compliance and completion of grant requirements

• Collaborate with Assistant Director of Client Services with evaluation, review and implementation of workflow processes.

• Assist Director of Client Services and Assistant Director of Client Services with programmatic needs, which may include site visits by funders, facilitating monthly department meetings, and reporting

• Abide by all state, federal laws and agency policies as related to confidentiality and Health Insurance Portability and Accountability Act (HIPAA).

• Assist the agency administration in the development of long-range plans, including goals and objectives, as directed by supervisor.

• Abides by all agency policies and procedures, including conflict of interest policy.

• Additional duties as assigned.

MINIMUM QUALIFICATIONS & EXPERIENCE:

• Master of Social Work or related masters in a related service delivery field (e.g. public health, human services)

• Minimum 3-year experience in social work or related service delivery field, with an emphasis on locating, coordinating and developing supportive services for identified clients across a broad

psychosocial continuum is required.

• Minimum of 2-year experience in an HIV care setting is required.

• Administrative or supervisory experience required.

• Fluency (written and verbal) in Spanish strongly preferred, but not required.

• Experience documenting in an Electronic Medical Record (EMR) system required
