We are seeking a Care Coordinator to support our clients and caregivers by ensuring care plans are executed at a high level. This role is ideal for someone who is organized, proactive, and enjoys building relationships with both clients and caregivers.

Key Responsibilities

  • Complete in home client visits to convert prospects and sign them up for services
  • Serve as a primary point of contact for clients and families
  • Conduct in-person or virtual supervisory visits
  • Conduct regular client check-ins and ensure satisfaction
  • Support caregiver onboarding to new cases
  • Monitor care quality and address concerns quickly
  • Coach caregivers on client care tactics
  • Partner with scheduling to ensure strong caregiver-client matches
  • Document all client interactions in our management tool
  • Participate in monthly on-call rotation'
  • Field work with clients across DuPage, Cook, and Will counties

  • Comfortable working in a fast-paced, service environment
  • Prior home care or healthcare experience preferred

Compensation

  • $22 – $27 per hour, based on experience
