Job Description

Find your purpose with CentraCare working as the Chronic Disease Management RN at our Melrose Clinic! The Chronic Disease Management RN renders care to patients in the clinic setting working as a valuable member of the health care team making independent and dependent decisions under the direction of a provider and in collaboration with a core team. In this role, you will coordinate a weekly review of caseload to develop plans that provide optimal care to the patient. The Chronic Disease Management RN is a co-leader in the core team, and is responsible for collaborating with services, both internal and external to the clinic to facilitate care transition to effectively meet the patient's needs by providing health education with a focus on self-management, prevention and wellness, based on the patient's goals. This position provides care to patients and families, supports and implements patient safety and other safety practices as appropriate.

  • Schedule
    • Full-time | 68 hours every two weeks | Melrose Clinic
    • 4 shifts per week | 8:00 am – 5:00 pm
    • No weekends or holidays
  • Pay and Benefits
    • Pay based on experience and degree
      • Associate/Diploma in nursing starting at $37.27 per hour
      • Bachelor’s in nursing starting at $38.56 per hour
      • Masters in nursing starting at $39.84 per hour
        • Pay range: $37.27 - $62.18 per hour

  • Qualifications
    • Graduate of an accredited school of nursing
    • Registered Nurse licensure in the state of Minnesota
    • Basic Life Support (BLS) American Heart Association (AHA) Healthcare Provider card
    • Minimum of 2 years of successful clinical experience, preferably in chronic disease education
    • Certified Diabetes Educator National Certification required within 2 years of employment
